Job description

Adult Future Options works with adults, predominantly aged 18-65, who have a physical disability or learning disability and are eligible for support under the Care Act.

You will work closely with these individuals, their families and support networks to promote the person's independence, whilst ensuring their health and wellbeing is maintained. The service supports people to live a fulfilling life at home, where they are connected to the community and resources around them.

The goals of service are to: 

  • Enable individuals to develop daily living skills and work towards increased independence i.e. their own tenancy in the community.
  • Work with carers, families, friends and advocacy services to ensure a person-centred approach is maintained.
  • Provide support or guidance to build and maintain healthy interpersonal and family relationships.
  • Encourage individuals to access their local community, with support or independently, to promote social inclusion with peers and the wider community.
  • Identify future life goals, meaningful activities and aspirations to work towards such as voluntary or paid employment.

Your main responsibilities will be completing annual reviews with people who ordinarily reside in residential or supported living placements. You will consider if the support they receive remains appropriate to their level of need and will commission or amend packages of care where required.

Using a strengths-based approach you will seek to empower the person to have more choice and control over their lives. You will prevent and manage safeguarding issues as they arise, with the support of social workers and other professionals, as part of a multi-disciplinary team. We are looking for applicants who are person-centred in their approach and who have excellent communication skills.

The role will include liaising with different teams and external providers, predominantly via electronic communication, requiring the postholder to be confident with IT systems.
