SalaryBCP Council are recruiting for an Adult Social Worker to work in the assessment team.
The Role:l
To provide Social Work support to the adult public focussing on people with long term health conditions undertaking statutory assessments, support planning and review of service.
* Assessment, support planning and review of referrals from the public
* commission & review BCP funded Packages of care
* Advice & guidance to support people to live healthy and independent lives
* Statutory safeguarding, risk management and court work
* Work with MDT's to provide joined up approach
* Manage own caseload
Essential Requirements:
* Social Work Qualification - Degree
* Experience as a Social Worker within Adult Social care teams
* Social Work England Registration
* Driving license and access to own vehicle
