This role has a starting salary of £66,774 per annum, based on a 36 hour working week.

This is a 12 month Fixed Term contract/ secondment opportunity.

The team support remote / agile working, however, your primary base will be Fairmount House, Bull Hill, Leatherhead, KT22 7AH. Regular travel during the week is required. For this reason, you must hold a valid driving licence

About the Team

Older People's Commissioning sits within the broader Adult Social Care directorate. Working closely with all locality teams, care providers, health colleagues and partners across Surrey our purpose is to improve the life opportunities of older people in Surrey. Ensuring no one is left behind and everyone has the chance to thrive at home or in care we want to ensure equity of access and opportunity and empower thriving communities.

About the Role 

With integration remaining a key priority for Surrey County Council and Heartlands and Frimley ICS we are seeking an innovative, experienced and transformational Senior Commissioning Manager to oversee implementation of some of our key work programmes and services.

The role will be recruited to Surrey County Council, Adult Social Care, and be based within the Older People's commissioning team however the role will be aligned to the Mind and Body Programme. The role will focus on bridging the gap between working age adult services and older people services, in particular prevention and hospital discharge.

Key areas of focus for the post holder:

  • Deliver our 'Reablement Fit for the Future' ambitions, focusing reablement on more equitable community referrals and hospital discharges through working with intermediate and primary care colleagues and social work teams across Surrey
  • Through discharge to assess (D2A) look to re-imagine our offer for residents with disabilities, mental health and older people for more effective long term care planning and support from new models of service delivery offering rehabilitation and reablement
  • Work with ASC transformation, commissioning and place leads to improve opportunities and outcomes for residents with more specialist and complex needs – from assessment to placement and beyond, work to improve mental health pathways, complex needs support (non-weight baring patients and delirium) and frailty pathways recognising what is already happening in the system and where improvements can be achieved through practice or new capacity conversations with care providers.
  • Improve the accommodation with care and support offer to have small scale specialist dementia residential and nursing care available to meet a range of needs
  • Support the 'whole system' approach that enables community mental health support to wrap around care home residents, with training for staff and support from community teams for residents that have behaviours that challenge

A key part of this role will be to identify areas for collaboration and integration, using existing work and governance to agree changes to processes and planning and where possible identify opportunities for new ways of paying for and delivering services seamlessly between health and social care.
