Job description

A Vacancy at Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust.
We are seeking an enthusiastic and self-motivated Advanced Cardiac Physiologist to join our excellent team in our Cardio-Respiratory Department.
Applications are welcomed from candidates with a degree in Clinical Physiology. Band 6 physiologists will considered for progression to band 7.
Successful Band 6 candidates will progress to band 7 upon successful completion of in house training and any relevant post graduation qualifications such as BSE, HRUK ect

The role will require you to work across all areas in the Trust which includes those that are designated as positive, asymptomatic and negative COVID wards.
At WWL, we have an excellent record for delivering first class patient care, throughout a wide range of invasive and non-invasive procedures, therefore the successful candidate will ultimately be able to perform a range of complex procedures as well as other routine procedures, including:
• Assist with angioplasty procedures
• Assist with angiograms procedures
• A working knowledge of basic Echocardiography
• Perform Cardiac Clinical Physiologists led exercise tests
• Advanced respiratory testing
• Perform Cardiac Clinical Physiologists led tilt tests
• Assist in Pacemaker Implants
• Pacemaker Follow up clinics both in person and remote, Bradycardia devices and complex CRTD and CRTP devices
• 24 Hour Tape Analysis
• Completing data input and performing basic cardiology and respiratory investigations
The post holder will be required to work as an independent practitioner and therefore must have the ability to effectively prioritise workloads, and maintain resilience when working under pressure. Successful candidates will be competent working with IT systems, and to the performance requirements of a busy department. BSE accreditation and/ or HRUK/ NASPE, ARTP would be an advantage.

Duties and Responsibilities
Key results from the job holder
Perform a minimum of two from the highly complex diagnostics 1-4
1. Perform echocardiograms, which evaluate patient’s heart structure and function, derived from 2 dimensional studies, colour flow, pulse wave, continuous wave Doppler and TDI. Desirably this should include congenital heart patients but with supervision when required.
Produce comprehensive written report to the British Society of echocardiogram recommendations. Communicate complex clinical information to the referring Consultant as and when required.
Supervision / assisting the Senior CCP when reporting, if required.
These all require specialist scientific knowledge acquired through professional training and in the specialist area of cardiac ultrasound.
2. Performing Pacemaker clinics and assisting in implant procedures with full reporting. Arranging pacing follow up clinics. Desirable to perform implant and follow up checks for CRT / ICD and pacing devices. Reporting on all aspects of the clinic and advising future treatment such as changing pacemaker parameters and arranging further diagnostics. HRUK or equivalent device training would be desirable. Accreditation in Advanced Life Support desirable but Immediate Life Support essential to act were necessary to defibrillate the patient during emergency resuscitation procedure.
1. Highly specialised Percutaneous Coronary Interventions. This involves monitoring of ECG, pressure, pressure wires and oxygen saturation of patients Interventions. Production of mandatory documentation of specific clinical information for the procedures and follow up of the patient in line with national protocols. Actively participate in life support and were necessary to defibrillate the patient during emergency resuscitation procedure. This will also include an on call rotation. Professional training in the specialist area of Percutaneous Coronary Interventions and Cardiac Catheterisation. Essential accreditation in Immediate Life Support, Advanced Life Support would be desirable.
1. Assist or perform all types of complex respiratory function testing including advanced full lung function testing (functional residual capacity, transfer factor, pharmacological challenge etc) and, Interpreting all respiratory test results, reporting and if the result are abnormal ensuring the referring physician is notified ARTP accreditation desirable
Essential complex and basic diagnostic testing
• Perform CCP led exercise test and CCP led tilt table testing which involves assessment of the referral, preparing and supervising the patient, monitor the patient’s electrocardiogram, take and record manual and automatic blood pressure. Interpretation of results, producing written report including making recommendations when appropriate and reporting back to the referring physician. Ensuring patient safety and determining the appropriate termination of the test. Taking patients history and where required informing the patient of the outcome.
Supervising and assessing junior CCP.
This requires specialist scientific knowledge acquired from formal and professional training in the specialist area of CCP driven service. Mandatory training and accreditation in either Advanced Life Support or Immediate Life Support.
• Assist doctor led exercise tolerance testing and pharmacological exercise tolerance testing by monitoring the patient’s vital parameters and record changes and make recommendations to the doctors when appropriate.
• Assist in pharmacological stress echocardiograms (with and without contrast medium), which are derived from rapidly acquired 2 Dimensional images that requires dexterity of a superior ability combined with professional training in the specialist area of pharmacological Stress Echocardiograms. Assist in the production of a comprehensive report that evaluates patient’s heart structure and function to give clear and concise clinical information for the Consultant
• Assist in the invasive procedure transoesophageal echocardiogram, and assisting in the production of a comprehensive report that evaluates patient’s heart structure and function to give clear and concise clinical information for the consultant.
• Analysing and reporting 24hour to 7 day ambulatory ECG monitoring, blood pressure monitoring and Event Recorder to produce comprehensive reports give clear and concise clinical information for the referring consultant. Interpretation of the report is mandatory to ensure the report is acted upon with urgency when required.
• Perform basic spirometry and peak flow. Interpreting all respiratory test results and reporting respiratory tests when results are abnormal to the referring physician
• Perform any other duties appropriate to grade as required e.g. clerical, data input, patient enquiries, stores and defibrillator checks
Planning and Organisational Duties
• Communicate complex information related to the service and development to the Cardio-respiratory Service Manager or Deputy Cardio-respiratory Service Manager and Cardiologists
• Assist in ensuring specialist clinical physiological policies are in place and adhered to.
• Responsible for patient and client care whilst undertaking diagnostic testing.
• Responsibility to ensure equipment used is checked for safety.
• Produce reports for some specialist activities undertaken within the department as required by the Service Managers.
• Freedom to Act as required within the established policies and codes of practic
Communications and Key Working Relationships
• Communicate service-related information to the senior Cardiac Clinical Physiologists, and MD
Responsibility for Finance

• Responsible for assisting in audits and research and developments as and when required at the decision of the cardiology managers
