Provide expert advice on perioperative care, including shared decision making and mental capacity Provide line management for the junior POPS team Service development Lead on the set up POPS pre-operative comprehensive geriatric assessment (CGA) clinics for older or frail surgical patients who need geriatric medicine support for planned surgery at Queen Elizabeth hospital Support in the business planning and recruitment of staff as the service evolves / expands Be point of contact for local and external teams seeking perioperative advice Work with local and external stakeholders to develop pathways of care to better prepare patients for surgery Link with external agencies, primary care and other community services to align perioperative practice and support the co-design of services to address health inequalities Clinical Service Out-patient clinic work Support the expansion POPS clinic on both hospital sites to provide comprehensive pre-operative clinical assessment using CGA methodology Facilitating an environment where shared decision making and, where appropriate, advanced decision planning can be discussed and documented. Liaise with surgical pre-assessment teams and waiting list managers to coordinate surgical admissions Identify, develop and deliver management plans to address pre-operative multidisciplinary goals Develop and initiate POPS Multidisciplinary team meetings to provide clinical supervision and intradepartmental learning. Surgical ward liaison Provide ward rounds to Review known POPS patients Deliver proactive case-finding of at-risk patients (emergency admissions) Deliver comprehensive geriatric assessment of those patients highlighted and work in partnership with the in the consultant perioperative specialists in delivering the prompt management of medical and surgical complications Develop and attend surgical ward MDT meetings to Interpret and analyse complex clinical and non-clinical facts to improve quality of care for older people and facilitate discharge planning Use expert theoretical and practical knowledge to refer on to other departments/services within or outside the Trust Apply prevention strategies for adverse outcomes such as readmissions and prolonged inpatient stay In addition to regular visits to the surgical ward the post holder will rapidly respond to acute issues and management of post-operative problems on the surgical ward liaise as appropriate with social services, intermediate care teams, primary care and voluntary agencies in order to implement the required services ensure high quality communication with the patient, relatives and carers support patients and carers through their surgical journey Have an awareness and understanding attend and contribute to LOS/Cluster meetings in Surgery Education Link with local practice development, surgical and perioperative care teams to provide education on frailty and comprehensive geriatric assessment methodology Provide ad hoc education and role modelling for the ward and surgical team as required Research Support the proactive data collection of patients seen by POPS to inform service impact and help drive service improvement. Have an active local research portfolio to support local service design and future business case development Support national audit and research to drive local and national best practice, linking with external agencies, stakeholders and services.
