We are looking for a highly skilled Advanced Practitioners to deliver effective clinical care to the patients within Venn PCN via our new home visiting service.
This is available as a Development Role for the right candidate.
This is a busy but extremely rewarding and vital role to support the housebound patients of VENN PCN to access safe and effective clinical care, in this role you will manage the care of a range of patients across all practices of Venn PCN.
• The post-holder will be required to complete system training and supervision to then work independently in the telephone triage of patients.
• After completing in-house competencies, the post-holder will be expected to work without direct supervision as an Independent Practitioner.
• After completing necessary clinical supervision, the post-holder will work as an independent practitioner providing face-to-face assessment, examination, diagnosis, treatment, management, referral, review or discharge, utilising a range of clinical skills and knowledge for patients presenting with undifferentiated and undiagnosed conditions, within the Competency framework.
