Advanced Clinical Practitioner
Salary: £52,481 - £59,071 per year FTE and DOE + Enhancements of - Saturday - midnight to midnight + weekdays after 8pm and before 6am = 30%, Sunday + Public Holidays = 60%.
Hours: Part time - Minimum of 1 day per week
Holiday: 25 days FTE (increasing to 27 after 5 years and 31 after 9 years) + public holidays
This exciting role will provide the post holder with the opportunity to work with a dynamic team, primarily within our 9 bedded end of life care unit. The post holder will join our growing senior multidisciplinary team to support palliative care to patients and their families. Prescribing ability is a core requirement of this post.
The Advanced Clinical Practitioner will use advanced practice skills to identify, assess, diagnoses, treat and manage patients with palliative needs. They will monitor and improve standards of care through patient centred goal setting in close collaboration with the internal and external multidisciplinary team. Using an up to date evidence base to supervise practice, audit clinical care and to provide skilled professional leadership. They will employ a proactive influence at multidisciplinary team meetings to enable effective care provision that is proactive, patient centred and innovative.
