Job summary

Mayfield Medical Centre is a training practice supporting 21,500 patients across three sites in Paignton and Brixham. We are looking for an individual to be part of our forward-thinking team committed to maintaining a sustainable and enjoyable work environment. We actively explore new ideas to enhance patient care. Main duties of the job

As an Advanced Nurse Practitioner at Mayfield Medical Centre, you will play a pivotal role in providing high-quality, patient-centred care to our community.

About us

We play an active role in Paignton and Brixham PCN with one of our Partners sharing the Clinical Director Role. We benefit from a full complement of Allied Health Professional roles, from our passionate Social Prescribing and Mental Health teams through to our Pharmacy team who play a key role in our medicines management. Our on the day team; consisting of GPs, ANPs and paramedics; look after acute presentations whilst maintaining continuity of care.

As an Advanced Nurse Practitioner at Mayfield Medical Centre, you will play a pivotal role in providing high-quality, patient-centred care to our community. Your responsibilities will include:

Conducting comprehensive assessments, diagnosing illnesses, and developing treatment plans for patients across their lifespan.

Prescribing medications, ordering diagnostic tests, and interpreting results to facilitate timely and effective patient care.

Collaborating with GPs, nurses, and other healthcare professionals to coordinate care and ensure continuity of services.

Educating patients and their families about health promotion, disease prevention, and self-management strategies.

Participating in quality improvement initiatives and contributing to the development and implementation of evidence-based practice guidelines.

Experience

Essential

  • Evidence of appropriate continuing professional development activity to maintain up to date knowledge and competence in all aspects of the ANP role.
  • Evidence of working autonomously and as part of a team.
  • Proven ability to evaluate the safety and effectiveness of their own clinical practice.
  • Experience of acute and chronic illness.

Desirable

  • Experience of working in primary care/GP practice setting
  • An understanding of the QoF framework
  • EMIS experience.

Qualifications

Essential

  • Full NMC Registration
  • Recognised Nurse Practitioner qualification at minimum degree level or equivalent
  • Independent prescriber

Desirable

  • Commitment to undertaking further qualification depending on practice need.
