Advanced Nurse Practitioner (ANP)

This is an exciting time for PC24 as we are transforming and implementing new models of care across our practices. We are seeking to appoint ANP’s covering our 12 practices and our new purpose-built primary care hub offering a family-friendly service to our patients across Liverpool and Sefton.
We also have vacancies for face-to-face ANP’s within our out of hours services with a variety of shift patterns.
Key responsibilities
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national/local protocols and within scope of practice.
- Work with patients to support compliance with and adherence to prescribed treatments.
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.
About you
You will be an experienced ANP within primary care who is able to deliver quality individualised care to our patients with a high level of autonomy underpinned by the four pillars of Clinical practice, leadership and management, education and research. You will be expected to utilise your skills in advanced clinical assessment and diagnostics reasoning to make complex decisions to determine and initiate changes to treatment. Flexibility, compassion and a can-do approach is essential for this position. The successful candidate will need to fulfil all the role requirements to be shortlisted for interview.
As a PC24 ANP you will have an MSc in advanced Clinical Practice
- Full NMC Registration with no restrictions
- MSc in Advanced Clinical Practice
- Non-Medical Prescriber – V300
- Clinical Examination and Diagnostics Level 7
- Experience of working in urgent care and/or primary care

Hours
Primary Care Shifts are available between the hours of 08:00 and 18:30 across all 12 of our locations in Sefton and Liverpool. We are a patient centred organisation and therefore require flexibility to cross cover cluster sites as required:
