The Community Cardiology Service provides individual, evidence-based care for patients who have been diagnosed with a recent myocardial infarction or heart failure with reduced ejection fraction (HFrEF). The service delivers clinic and home visiting care to patients whose GP sits within the boroughs of Ashton, Wigan and Leigh. The purpose of the service is to educate, support, and improve the quality of life of those diagnosed with relevant cardiac conditions and to prevent unnecessary hospital admissions working collaboratively with key stakeholders. The job holder in partnership with members of the multidisciplinary team will be expected to initiate and provide evidence-based care, provide expert advice for patients and their carers with complex cardiac conditions, providing relevant treatments as required. This role as ANP requires the post holder to provide support and guidance for junior members of the team. The post holder will lead on clinical audit activity within their specialist area where they will contribute / lead the ongoing development of clinical practice, standards of care within the service including the development of policies, procedures, protocols, and guidelines. The post holder is accountable for their safe, effective clinical practice and working as part of a defined team and supporting team performance, ensuring efficient and effective use of physical and human resources.
