To contribute to the provision, development and maintenance of a comprehensive, high quality clinical and technical pharmacy service to patients requiring chemotherapy To cross cover with other senior pharmacists in the team in their absence as appropriate when required. To review and approve prescriptions, procedures, guidelines, reports and other documents (electronic and paper based) for accuracy & appropriateness. Provide clinical pharmacy expertise to improve patient centred care of cancer patients; this may include clinic based activities such as medication reviews, prescribing, and patient counselling. To assist in the implementation and maintenance of chemotherapy electronic prescribing to haemato-oncology. Liaise with the pharmacy teams within the aseptic unit and outpatient dispensing pharmacy to ensure timely supply of chemotherapy for haemato-oncology patients. Support the haemato-oncology pharmacy team with service development projects The post-holder will be part of a fabulous haemato-oncology pharmacy team.
