Please see attached Job description for full details Work as part of the multidisciplinary team working under the direction of AnaemiaNurse Practitioner/ Renal manager/Matron. In collaboration with the Anaemia Nurse Practitioner, develop and deliver theanaemia services, providing a Nurse Led Clinical Anaemia Service, whichincorporates, the assessment, counselling, education, and monitoring of patientsrequiring ESAs, HIF inhibitors and /or iron. Monitor treatment, compliance and advise on modifications to treatment withinagreed guidelines. To provide advice, training and support to enable appropriatepatients to safely administer ESAs and HIF inhibitors. Continue to develop advanced knowledge and skills within the specialist sphere ofpractice. Continually reappraise the service provided to patients on ESAs HIF inhibitors andiron therapy, acting as a catalyst for change and improvement. Support with the planning, delivery and evaluation of care pathways for a designatedgroup of patients under the direction of the Anaemia Nurse Practitioner/Manager/Matron. Support the development of nursing practice within the specialist field through theuse of evidence-based practice, in line with agreed national standards and contributeto the body of nursing knowledge. Work as an autonomous practitioner within their sphere of practice. Continuously improve clinical standards, in line with clinical governance framework. Exercise accountability, as set out in the NMC Code. Contribute to training and education within the specialist area. Provide specialist clinical advice and support to patients, their families and clinicalstaff within the Trust on the management and care of patients within the specialistarea. Provide direct access for patients, either face to face or telephone consultation. Represent the service nationwide in the absence of the Anaemia Nurse Practitioner/Manager/Matron. Play a key role in ensuring that national and local standards/targets for the specialistarea are met. Manage and lead nurse-led clinics in relation to reviewing patients and play a keyrole in optimising complex treatment regimes, facilitating hospital discharge andpreventing avoidable hospital admissions. Support and undertake research relevant to the needs of the specialist practice. Comply with the Trusts vision, values and behavioural compact
