Job description

Are you a thoughtful, dedicated, dynamic nurse who is passionate about holistic patient care?

An exciting opportunity has arisen, to appoint an Associate Community Matron based in Bath and North East Somerset! 

As an Associate Community Matron you would work closely with the Matron, Primary care providers and other members of the Multidisciplinary team to support patients in the community setting who are vulnerable and have complex healthcare needs. You will case manage your own group of patients, with the support of the Matron, providing clinical assessments and intervention for adults in the community. 

This is a flexible part-time position working 30 hours across the week including potential evening or weekend work to align with the Urgent Care Response model.

Main ResponsibilitiesAs an Associate Community Matron you will:


Work autonomously as an Associate Community Matron to identify and manage patients with acute / intensive medical needs
Provide clinical expertise with the support of the Matron on leadership for the team
To be support the co-ordination of weekly Multi-Disciplinary Team Meetings (MDTs)
To case manage identified patients who require intensive community support
Recognise the early symptoms of disease exacerbation and acute illness
Be responsible for the decisions about the care and service provision, for individual patients requiring intensive community support
Have managerial responsibility for an Assistant Practitioner/Nursing Associate or Locality Care Co-Ordinator as required

The Ideal CandidateEssential:


NMC Registered Nurse and have studied at degree level
Full UK license and have access to a car for work purposes
Confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.

Management/leadership Experience
