Job summary

We are looking to recruit an enthusiastic and efficient flow co-ordinator to join the oncology/haematology teams. The position will be highly demanding but very rewarding with the ability to offer a person-centred approach to a patient's admission, discharge, or transfer. The post holder will be expected to support the senior nursing team as well as maintaining a regular presence on the ward, attend handover / meetings and obtain information to enable clinical / operational teams to make informed decisions about patient flow relating to planned and unplanned referrals and discharges. The post holder will also require excellent communication skills and the ability to interact extensively with patients, relatives, and the multi-disciplinary team. The post holder will also be responsible for updating 6th floor report, SMA and ward view and also be responsible for ensuring that our service users' needs are met through the delivery of high-quality care and information. The post holder will work an agreed shift pattern (varying) over a 7-day working week.

Main duties, tasks & skills required

1.Collaboratively communicating and working with MDT team.

2.Record /coordinate and communicate details of patients being: a. Admitted b. Discharged home or to another hospital / practice setting, and inter hospital transfer.

3.Has awareness of the following at ward level : a. IPC status of patients, ward need for different cleaning regimes b. Side room use / potential c. Monitoring provision ie : does the patient need specific care treatment, d. Potential /actual discharges / transfers in out / Pre-lunch discharges e. End of life care needs f. Vulnerable patients requiring consideration due to mental health / learning disability /or hearing needs.

4.Establish robust processes with all involved in the service patient pathways to negotiate and improve the co-ordination of the patient journey.

5.Assist in the data entry/development and maintenance of databases/ electronic patient records to capture patient information, monitoring information by proactively identifying and taking actions to enable patients to be prioritised, admitted/discharged as per individual management plan.

6.Support the nurse in charge of the service/ ward / unit to monitor and ensure the most effective use of beds within the service.

7.Monitor appropriate admissions and in patients progress on a daily basis and prevent discharge delays wherever possible and escalate accordingly

8.Have responsibility for the timely and accurate data entry of patient information.

Qualifications

Essential

  • Good General Education (eg. GCSE English and Maths A-C GCSE LEVEL 9-4
  • Business Administration NVQ level 3 or equivalent experience in an Administrative environment

Experience

Essential

  • Experience of dealing with the Public/Customer service experience
  • Experience of working in a Secretarial/ Administrative role with a proven track record of problem solving
  • Experience of dealing with the Public/Customer service experience
  • Good Organisational skills
  • Able to use own initiative and deal with the unpredictable
  • Able to work under pressure and to multitask
  • Able to work to deadlines

Desirable

  • Experience of working in a busy environment
