Job description

A Vacancy at Birmingham Community Healthcare NHS Foundation Trust.
Community-based - will need driving licence & access to a car.
Suitable for newly qualified nurses.

We are looking for a group of highly motivated applicants who are interested in meeting the challenge of working on the community with patients in the home setting. You will be working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients to ensure their independence and well being. The post holder will carry out a variety of nursing duties as part of the patients holistic care management through the assessment and evaluation of health needs particularly in the areas of palliative care, wound management, diabetes and other long term conditions.
Vacancies are across West Locality Planned Teams.
The post holder will work as part of the integrated Multidisciplinary team under the supervision of the Clinical Team Leader. He / She will be accountable for the delivery of high quality care to patients within their own home at GP premises/health centres or other community setting. Working with other members of the IMT to provide care to and promote health/well being and independence for patients on the team caseload, working closely with other partners/stakeholders including GP’s Rapid Response and Advance nurse practitioners in the prevention of inappropriate hospital admissions and facilitation of hospital discharges in line with service objectives.

BCHC is obliged to comply with this statutory obligation. Where you are applying for roles where some or all of your potential duties fall within the scope of the health and social care mandatory vaccination regulations, you should be aware that it will be a condition of your employment with BCHC that you have received the 1stand 2nddose of the Covid-19 vaccination and have provided evidence of this (unless an exemption applies) by the relevant dates, being 3 February 2022 for the 1stdose and 31 March 2022 for the 2nddose. If you are medically exempt from vaccination, we require evidence of formal medical exemption through the NHS COVID Pass process before BCHC can confirm acceptance of your medically exempt status. If you would like further details on the new rules around mandatory vaccinations, these can be provided during the recruitment process.
Training and development will be provided to support successful candidates in achieving competence in these areas.
The service operates between the hours of 8 am to 6pm over 7 days per week and the working pattern for the successful candidate will include weekend working and bank holiday periods. This post requires the successful candidates to be highly mobile so requires a full driving license and car available for work.
