SalaryBand 5 District Community Nurse – Fast Track Compliance
Team Support Healthcare are currently recruiting District Community Registered Nurses to provide community and hospital support for our clients in Liverpool.
Location – Liverpool
Hours – Full-time and part-time hours available
Pay rate – Up to £30 an hour
Main Duties/ Responsibilities
- Deliver clinical care to a defined population.
- Provide care to patients who have recently been discharged from hospital and require additional support.
- Recognize, assess, and refer patients presenting with physical or mental health needs.
- Perform comprehensive assessments of housebound patients nursing needs.
- To work effectively and collaboratively within the team to ensure effective and safe care is delivered to assist timely hospital discharge and prevent re-admission.
- Collect and report information, maintaining accurate patient records in line with NMC recommendations and Trust policy.
- Communicate effectively with patients and carers, recognizing the need for alternative methods of communication to overcome different levels of understanding, cultural backgrounds and preferred ways of communicating.
Essential requirements
- Be a NMC Registered Nurse
- Must hold a full UK driving license with access to a car
- 6 Months recent relevant UK experience within the last 3 years.
- You will need to pass an enhanced background check.
