Job Title: Caseworker (RMN/SW/OT) - Band 6Intensive Support Team

Job Description:
We are currently seeking a highly motivated and skilled Caseworker to join our Intensive Support Team at Band 6 level. The Intensive Support Team provides specialized care and assistance to individuals facing complex mental health challenges, including those with co-occurring learning disabilities and autism. As a Caseworker, you will play a crucial role in delivering comprehensive support services, working collaboratively within a multidisciplinary team environment.
Main Duties of the Job:
* Provide direct support and interventions to individuals with complex mental health needs, learning disabilities, and/or autism within the community.
* Conduct comprehensive assessments, formulate care plans, and implement evidence-based interventions to address behavioral challenges and promote recovery.
* Work closely with service users, their families, and carers to develop and implement person-centered care plans, ensuring individual needs and preferences are considered.
* Collaborate with other professionals, including psychiatrists, psychologists, social workers, occupational therapists, and community support workers, to coordinate and integrate care services effectively.
* Actively participate in team meetings, case conferences, and clinical supervision sessions to review cases, share insights, and contribute to ongoing service development.
* Maintain accurate and up-to-date records/documentation, adhering to relevant policies, procedures, and confidentiality guidelines.
* Provide crisis intervention and support during acute situations, liaising with emergency services and other relevant stakeholders as required.
* Engage in continuous professional development activities to enhance clinical knowledge and skills, keeping abreast of best practices and emerging trends in mental health care.
Requirements:
* Qualified Registered Mental Health Nurse (RMN), Social Worker (SW), or Occupational Therapist (OT) registered with the appropriate regulatory body.
* Previous experience working in mental health services, ideally within a community or Intensive Support setting.
* Sound knowledge of mental health legislation, risk assessment, and care planning processes.
* Strong interpersonal and communication skills, with the ability to build therapeutic relationships and engage effectively with diverse individuals and groups.
* Demonstrated ability to work autonomously as well as collaboratively within a multidisciplinary team environment.
* Flexibility to adapt to changing priorities and work effectively under pressure.
* Commitment to promoting recovery-oriented, person-centered care principles.
* Valid driver's license and access to a vehicle for travel within the community (where applicable).
