Band 6 Clinical Nurse Specialist Dementia

Full time 37:5 hrs Monday to Friday (0800-1600)

An exciting opportunity has arisen for a Registered Nurse Adult, to join the Vulnerabilities Team. The Vulnerabilities Team is a passionate and dedicated team of Learning Disability, Mental Health, General nurses, and Dementia Specialists Nurses; we are looking for an enthusiastic, passionate, and motivated RGN who has acute experience working with patients who have Dementia. To work as part of the team in shaping and transforming acute hospital care and experience for patients with Dementia

The case load is diverse and we work Collaboratively to meet the health needs of this group of individuals attending the hospital, either as an inpatient or outpatient to ensure that their holistic care needs are met, by providing and supporting specialist intervention, working together by adopting a person centered approach to optimize health and wellbeing, we strive to address health inequalities and empower individuals to have independence , choice and control

The post requires a flexible approach and requires the individual to work across the three sites depending on service demand/Pressure.

Main duties, tasks & skills required

The role is diverse; and requires the post holder to review and assess patients, liaise, support and teach staff, implement new ways of working, support reasonable adjustments, work collaboratively with individuals, families, and carers when planning admissions, during inpatient care through to discharge to ensure a positive experience.

You will be required to work Trust wide/ across all sites, acting as a role model, sharing expertise across multi-disciplinary teams and work collaboratively. Therefore, you will need excellent communication, organizational and interpersonal skills, you will need to be able to work as part of a team or on your own. You will need to be resilient to an ever-changing workload be able to prioritize and manage the workload, and ensure a high standard of clinical care is delivered to the patients under our care

We welcome applicants who have a current NMC registration RGN, with knowledge and experience of working with patients who have Dementia.

There are 4 main inpatient sites across the Trust and you will be expected to travel between sites, therefore a car driver is essential.

Qualifications

Essential

  • Registered Nurse on the NMC Register
  • Evidence of post registration CPD / Post registration qualification in appropriate area of speciality or professional knowledge supplemented by specialist experience and short courses to FHEQ Level 6

Desirable

  • Post registration qualification in Dementia care

Experience

Essential

  • Evidence and ability to revalidate as required by the NMC
  • Significant post registration experience. with evidence of passion/ enthusiasm and knowledge for Dementia Care
  • Evidence of achievements and career development in current post and of ongoing professional development/ competence.
  • Demonstrate specialist knowledge underpinned by theory and CPD relevant to Dementia
  • Experience and enthusiasm for teaching / assessing and supervising others in a clinical setting
  • Experience of working in a multi-disciplinary and cross agency work environment
  • Can demonstrate understanding / experience / willingness to be a practice based supervisor / assessor as detailed in the Nursing & Midwifery Council ( NMC ) standards for student supervision and assessment
  • Awareness of current relevant NHS policy context relating to the speciality
  • Has understanding / experience of the principles of Safeguarding in relation to the role applied for
  • Knowledge of corporate and clinical governance

Desirable

  • Experience in service development
  • Awareness of current research related to the speciality
