Job description

We’re looking for a passionate and dedicated Band 6 Community Nurse to join our friendly and welcoming Nursing team covering Garden City in Dartford North Kent to help us to continue to change things for the better.

Working closely with our Community Nurses, GP's and other Health care professionals, you’ll be working within a multi-disciplinary team responsible for contributing to the holistic assessment, planning, delivery and evaluation of care to patients within their own homes. You will be expected to implement, monitor and maintain high standards of care delivery at all times. You’ll care for patients and their families in the way that you believe is best, delivering our high service standards, sharing best practice, and actively improving the way we work.

This is a part time position (22.5 hours per week) with 9am - 5pm shifts. 

A full UK driving licence and access to a vehicle for work purposes is essential.

Main ResponsibilitiesAs a Community Nurse you will care for patients and their families in the way that you believe is best, delivering our high service standards, sharing best practice, and actively improving the way we work to help us continue to change things for the better. The Ideal CandidateYou will be a NMC/1st level registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. It is expected that you should have strong communication and organisation skills and a flexible approach which will ensure you deliver positive experiences to patients at every stage of their healthcare journey.  You will have a full UK driving license with access to a car for work purposes.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.

As this is a community based role a valid UK driving licence is required.
