Job Title: Band 6 Community Psychiatric Nurse (CPN)

Location: Chorley, United Kingdom

Contract Duration: 3-6 months, Full-Time

Salary: £25 per hour (via umbrella company)

Hours: Monday to Friday, 9:00 AM - 5:00 PM

About Us: We are currently seeking a dedicated and experienced Band 6 Community Psychiatric Nurse (CPN) to join our Community Mental Health Team (CMHT) in Chorley. Our team is committed to providing high-quality care and support to adults in the community who are experiencing mental health challenges.

Role and Responsibilities: As a Band 6 CPN, you will play a crucial role in providing holistic care to adults within the community setting. Your responsibilities will include:

• Conducting comprehensive assessments of individuals' mental health needs.
• Developing and implementing personalized care plans in collaboration with service users and their families.
• Providing therapeutic interventions and support to individuals, promoting recovery and well-being.
• Liaising with other healthcare professionals, social services, and community organizations to ensure coordinated care delivery.
• Participating in multidisciplinary team meetings and case reviews to evaluate and adjust care plans as needed.
• Providing education and support to service users and their families regarding mental health issues, treatment options, and community resources.

Requirements:

• Registered Nurse qualification with a valid NMC PIN.
• Strong assessment and care planning skills.
• Excellent communication and interpersonal skills, with the ability to build rapport with service users and collaborate effectively with colleagues.
• Ability to work autonomously and as part of a multidisciplinary team.
• Knowledge of relevant legislation and best practice guidelines in mental health care.
• Some experience in the community is preferred but not compulsory. We welcome applications from CPNs with varying levels of experience, including those transitioning from other healthcare settings.
• The successful candidate will be supported to manage a small caseload of individuals living in the community with an array of mental health needs. You will receive ongoing supervision and training to ensure you are equipped with the necessary skills and knowledge to excel in this role.
