Job summary

The Urgent Community Response Team aims to deliver holistic assessments and interventions to individuals who are at risk of hospital admission within their own home within 2 hours of referral to the service. The aim of the team is to enable people to stay at home, recover, and re-engage with the important aspects of their life and prevent admission to acute sectors.

The post holder will ensure access to Urgent Health Assessments, provide and monitor short term treatment programmes at home and ensure safe discharge or referral to long term health and social care interventions. Utilising clinical expertise, clinical judgement, discretion and decision making in the development of individualised treatment plans.

To work in partnership with primary, secondary and other health and social care providers, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care.

The post holder will be expected to work within a flexible working pattern over a 7 day week in the urgent community response team as required. Able to meet the travel requirements of the post and be independently mobile and as such it is essential for you to possess a full category B car driving licence preferably clean or with no more than a maximum of 3 points

Main duties, tasks & skills required

Act as an expert practitioner, professional role model and specialist resource to the multi-disciplinary team regarding UCR /Virtual Ward.

  • To facilitate early supported discharges from Acute hospitals.
  • Work alongside the care co-ordination centre to facilitate discharges to community services.
  • Facilitate teaching and supervision for less experienced members of staff and students
  • Promote health by collaborative working with patients and their relatives. Act to and facilitate others to respect the dignity, wishes and beliefs of patients; involving them in shared decision making and obtaining consent prior to all interventions.
  • Safeguard individuals from abuse by reporting any incidents involving potential or actual abuse and acting upon concerns immediately.
  • Participate in the further development of an integrated, multi-disciplinary Urgent Community Response Service working alongside colleagues in Integrated Multidisciplinary teams and the wider health and social care economy.
  • Participate in and lead clinical meetings to facilitate the continuity of patient care ensuring patients and relatives are kept informed of treatments, progress, and discharge arrangements
  • Participate in proactive service user engagement encouraging feedback and evidence actions taken in response to feedback received.
  • Completing advance care planning for palliative patients, including respect forms.

Qualifications

Essential

  • Relevant clinical qualification with current registration (eg RGN
  • Health assessment course
  • Relevant Degree level qualification or equivalent experience.
  • Minimal 4 years post registration experience
  • Evidence of continuing professional development
  • Mentorship qualification

Desirable

  • Non-medical prescriber

  • Experience of working in a multi-disciplinary team setting
  • Experience of comprehensive clinical assessment of patients
  • Experience of caring for patients who are acutely ill/require rehabilitation plan/end of life care
  • Experience of effective caseload management
  • Experience in line management and supervising junior staff

  • Experience in collection , collation and analysis of data for clinical audit and research

  • Demonstrate achievement of numeracy (including drug administration calculations) and literacy skills. Ability to read / write and speak , receive and understand instructions in English
  • Good IT skills ability to record information electronically
  • Competent in a range of clinical skills willingness to undertake expanded practices relevant to the scope of practice required
  • Competent in managing the care of a group of patients / shift management and clinical leadership relevant to the practise setting
