Job description

We have an exciting opportunity for a motivated and dedicated Band 7 Stroke Nurse to lead our new Early Supported Discharge service, working with our dynamic team at Gravesham Community Hospital. As a member of the Early Supported Discharge Team for Stroke patients and other Neurological conditions, you will play a crucial role in delivering high-quality care within the community setting. This is an excellent opportunity for a passionate nurse to make a real impact in the lives of patients and contribute to the ongoing development of our service.

Gravesham Community Hospital is located a short distance from St Georges Shopping Centre and is surrounded by well-known food establishments and local coffee shops making it an ideal location to be based from.

This is a full time position. The Early Supported Discharge service operates 8am - 4pm Monday - Saturday.

A full driving licence and access to a vehicle is essential in order to carry out community visits to patients in their homes and visit other bases.

Main Responsibilities
Responsible for clinical assessment of patients within the community setting with a focus on the Early Supportive Discharge Team for Stroke patients and other Neurological conditions.
To ensure understanding/compliance of medication, secondary prevention education.
Liaison with other community providers such as the GPs, community nursing staff. 
To provide Nursing Assessments within the community setting and in reaching into the inpatient setting (Sapphire Unit) and supporting the community neuro rehab service
To manage, supervise and develop other nurses, Rehab assistants, apprentices, and students.
To promote the role of Specialist Nursing within the inter-professional team and ensure that good communication exists with all staff in HCRG care group and outside agencies.
Actively monitor and develop the Nursing service within CNRT in line with best practice, continuously striving to improve quality of service.

The Ideal CandidateThe ideal candidate will have / be:


A Degree or diploma in Adult Nursing
Evidence of updated post-graduate training in relevant area of speciality to degree or equivalent level
A current NMC registration.
Long Term conditions management.
Evidence of employment as working in the community setting or evidence of employment as a Band 6 grade for a minimum of 2 years within the last 3 years.
Experience of working with Stroke patients
Experience of working with professionals from other disciplines
Experience of working with complex neurology patients
Knowledge and understanding of Stroke NICE guidelines
