Job description

To manage and take overall responsibility for care quality performance and clinical activity by providing professional nursing leadership and guidance, clinical advice and management in relation to the key dimensions of health care quality: Patient safety The effectiveness of all health care interventions Patient experience The environment in which care is delivered To ensure the delivery of a clinically effective service which meets the needs of individuals, families and communities in line with local and national development plans, Trust policies and procedures. The post holder will provide good leadership and management, to develop and sustain a positive working culture. The post holder will ensure staff are trained appropriately to deliver the care required and ensure supervision and mentorship is in place. To work collaboratively with other senior nurses within CAMHS LDT and wider CAMHS appropriate and promote the role of their speciality nursing to all stakeholders.
To foster a culture of networking in an open and transparent approach with internal and external MDTs and between all stakeholders. Clinical Responsibilities To lead and facilitate the clinical nursing component for Outreach Service. To put into effect Authority, Trust and Service Policies and Procedures and maintain them in the course of duty, advising all staff of changes. To monitor and develop, with the clinical / Operational Team leaders and the MDT, policies and procedures appropriate to the specialist area.
To organise work in such a manner as to ensure the delivery of the highest attainable standard of nursing care to clients in whatever setting and to ensure that each member of staff is aware of his/her duties and carries them out as required. To act as a source of advanced clinical knowledge and skills for junior staff. To ensure nursing participation in the discussion of all new referrals to assure that a nursing perspective is brought to bear on discussions made regarding their assessment and management. Plan and implement effective evidence-based nursing intervention through the problem solving approach of the Nursing Process and within the framework of the multi-disciplinary team approach underpinned by Positive Behaviour Support.
Evaluate the effects of nursing and MDT intervention, re-assessing and consulting and re-planning as necessary, adapting to the changing needs of the patients. Seek appropriate clinical supervision and to provide clinical and line management supervision for nursing staff. Undertake casework consultation or supervision with other specialist team members / professionals as appropriate. Make decisions on suitable locations for implementing assessment and intervention, e.g.
in Rothesay, home, school or other community based activities. Ensure that confidentiality is maintained on all matters relating to the private affairs and treatment of patients and their families. Be aware of and ensure observation by all staff of the legal requirements of relevant legislation Be aware of and ensure adherence by all staff to the Leicestershire Interagency Child Protection Procedures. Undertake work within patients' own homes and act as a lead professional for outpatient patients as allocated.
KEY RESULTS AREAS: Effectively line manage the nursing component of the Outreach service. Exercise advanced clinical nursing skills in partnership with other professionals. Provide a therapeutic environment for children and young people that is based upon risk assessment. Ensure the development of nursing, other staff and learners.
Maintain good communication and liaison with all members of the multi-disciplinary team, wider CAMHS and other agencies. Co-ordinate the planning and delivery of therapeutic programmes within the outreach service Provide relevant and high standard training to the nursing staff and other professional groups. Actively participate in future planning of developments for the CAMHS LDT Managerial Responsibilities Manage the Outreach Service and the delivery of nursing care. Manage the nursing component of the outreach service.
Facilitate and participate, as a multi-disciplinary team member, in the setting and review of aims, objectives, standards, strategy and therapeutic approach of the Service, ensuring that these are in accordance with the general policies of LPT NHS Trust, ICC and the NMC and remain relevant to the needs of the referrers/service users. Encourage the recognition that all staff are valued and are to be treated with respect and concern by their colleagues, recognising and appreciating good work and accepting constructive criticism with good grace. Ensure the maintenance by all staff of accurate records of the nursing process and of a high standard of verbal nurse to nurse/nurse to MDT reporting, in accordance with Trust Policy. Ensure high standards of liaison and communication with the clinical/operational team leads, the nursing team, multi-disciplinary team and other relevant agencies.
Monitor staff sickness and absence and take any appropriate action in accordance with the policy Ensure awareness and adherence by staff to the Authority and Unit policies for the ordering, dispensing, storage, prescription and administration of drugs. Ensure the recording and reporting of all complaints, accidents, untoward incidents and occurrences in accordance with Trust/Authority Policies. Take responsibility for the deployment of staff, ensuring the maintenance of staffing levels at, a safe functional level - communicating any problems arising from maintaining cover to the clinical/operational team leads. Deploy resources, labour and materials, etc., with a view to cost effectiveness.
Monitor relevant budgets, as delegated. Personnel Responsibilities To exercise leadership to achieve high morale based on sound organisation, fair and clear staff relationships and to promote good communication within the team by example To facilitate and be involved with the induction/mentorship of staff new to the Service. To convene and attend any Nursing, Clinical or Professional meetings as appropriate/allocated. To participate in staff development and personal development plans.
To ensure awareness of and adherence by staff to Authority and Trust Policies on Health and Safety, Employees responsibilities under the Health and Safety at Work Act 1974, maintaining a high standard of safety and safeguarding the welfare of staff and clients at all times. To ensure that Fire Drill Procedure is understood Educational Responsibilities To facilitate the provision of in-service training opportunities for the nursing and, where appropriate, the multi-disciplinary teams. To actively seek to enhance ones knowledge and skills by taking and making opportunities to, e.g. participate in in-service training course, seminars, therapeutic activities with which one is unfamiliar, reading relevant publications, etc.
To monitor the provision of and act as supervisor/assessor/mentor to students and new staff as required. To share and encourage the sharing of any special skills or knowledge acquired with colleagues to enhance the expertise of the team. To offer training and act as a resource to other health professionals and families Research and Quality Responsibilities To collect, and cause to be collected, accurate activity data to facilitate audit and the measurement of quality. To participate in research programmes within the guidelines of Leicestershire Health Authority Ethical Committee.
To evaluate individual care plans and measure standards of patient care To take a leading role in audit and the setting and monitoring of standards to assure quality. Physical demands Due to the diverse needs and difficulties of the patient group the post holder may be required to undertake the use of safe holding of patients for long periods of time The post holder may need to stand for long periods of time when undertaking assessment observations and training of others EFFORT AND ENVIRONMENT Mental Effort The undertaking of observations of patients for long periods of time Undertake the evaluation of information gained through assessment and compose subsequent reports and treatment plans Emotional effort required in the job The post holder will be required to undertake disciplinary measures as required with staff The post holder will be party to information that may be of a very sensitive nature The post holder may receive information that requires safeguarding procedures to be activated Working conditions of the post: The post holder will encounter episodes of verbal and physical aggression directed at themselves and others The post holder will be required to work within settings outside of LPT including family homes Please read attached generic band 7 Job description alongside this information
