Care Co-Ordinator
Overview:To provide accurate rotas for all staff in connection with provision of reliable and responsive person-centred care and support to people in their own homes and to play a supportive role to Team Leaders as part of the management of a high quality support service for older adults.
Main Duties:
- To co-ordinate staff rotas in order to fulfil service user needs.
- To co-ordinate training needs of all members of staff and ensure that the Care Co-ordinator and all staff with whom they work are familiar with Care Force’s Policies (including and especially the Confidentiality Policy)
- To co-ordinate and deploy staff resources as efficiently as possible in relation to the needs of service users and the requirements of the service in such a way as to ensure maximum efficiency and effectiveness in the use of staffing resources including continuity of carers, adherence to times requested by (and to suit) service users.
- To plan ahead to meet the needs of service users as identified by their individual Person-Centred Plan and communicate with service users with regard to any changes in available times and available carers.
- To follow guidelines and procedures and to be always professional and accountable.
Requirements:
- Good verbal communication skills and ability to listen sensitively to others.
Good written communication skills, with an ability to contribute to a record keeping system
Reliability and promptness with good time management skills
Clean driving licence and access to car
