Stevenage Healthcare Ltd (SHL) and Stevenage South Primary Care Network (SSPCN) has a fantastic opportunity for a care co-ordinator to join PCN, on a full time basis for 37.5 hours per week, to support our 4 GP practices across the Stevenage area, to assist in re-shaping primary care across our communities.
Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those living in Care Homes, to ensure appropriate support is made available to them and their carers, supporting them to understand and manage their condition and ensuring their changing needs are addressed.
This is achieved by bringing together all the information about a person’s identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person.
The main duties of the role include:
- supporting practices, where required, with day-to-day tasks, answering phone calls and responding to various admin and digital tasks.
- supporting the PCN to achieve admin-related contractual indicators as set out in the DES and IIF contracts.
- developing a strong knowledge and understanding of the PCN roles and services in order to facilitate positive conversations with patients who may benefit from specialist support.
- working with and provide training and information for practice-based staff to help practice teams increase their knowledge and understanding of the various roles and services available through the PCN.
- helping people to manage their needs through answering queries, making, and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care.
- actively referring patients into a range of services, based on their individual needs and requirements.
- providing coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health wellbeing coaches and other primary care professionals.
- supporting the coordination and delivery of MDTs within the PCN where appropriate.
- working with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients.
- supporting the clinical director and PCN members in the delivery of the DES contract.
- helping people to manage their needs through answering queries, making, and managing appointments.
- providing coordination and navigation with the aid of digital tools for people and their carers across health and care services.

About the Candidate
If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.
The ideal applicant will have the following skills, experience and qualifications:
- Experience of working in health, social care and other support roles in direct contact with people, families or carers (in a paid or voluntary capacity).
- Experience of working within multi- professional team environments.
- Experience of supporting people, their families and carers in a related role.
- Experience of data collection and using tools to measure the impact of services.
- Knowledge of the personalised care approach.
- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on
- communities, individuals, their families and carers.
- Understanding of the needs of older people / adults with disabilities / long term conditions particularly in relation to promoting their independence.
- Basic knowledge of long term conditions and the complexities involved: medical, physical, emotional and social.
- High level of written and verbal communication skills.
- Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines.
- NVQ Level 3 in adult care - advanced level or equivalent qualifications or working towards.
- Experience of working directly in a care coordinator role, adult health and social care, learning support or public health / health improvement.
- Experience or training in personalised care and support planning.
- Knowledge of Safeguarding Children and Vulnerable Adults policies and processes.
- Ability to travel across the locality on a regular basis.
About Us
Stevenage South PCN (SSPCN) is made up of local practices who are commissioned to work together to provide a range of healthcare services to a population of 60,000 patients across Stevenage. The core network practices include Bedwell Medical Centre, King George and Manor House Surgery, Knebworth & Marymead Medical Practice and Shephall Health Centre, however other practices may join the PCN in the future. The PCN is governed by a PCN board, with representatives from each practice, who provide oversight of the service and approval for strategy and future PCN plans and objectives. SSPCN have a fantastic work ethic and strongly believe in supportive teamwork.
Stevenage Healthcare Ltd has been tasked with providing delivery and support for Stevenage South Primary Care Network. The company will in directly employ staff to provide development and delivery of SSPCN contracts and areas of work, as well as explore wider healthcare opportunities to support local people and provide positive health outcomes for our population. This role will in future be transferred to SHL.
