Care Manager needed with Harrow CouncilStartDate- ASAPLocation - HarrowDuration - Initially 4 months Hoursperweek - 36 hours a weekSalary - GBP21.38 ltd an hourDescription:

• To provide professional fieldwork service to service users and their carers and to manage the needs of adults with physical, learning or sensory disabilities including those who are terminally ill
• To ensure that appropriate assessment of need is undertaken, and that care packages and other services are organised, monitored and reviewed to meet the needs identified within the resources available and to promote independence
• To assess for and provide equipment and minor adaptations in order to promote independence, and to monitor these as necessary
• To maintain and take responsibility for effectively managing a personal workload in consultation with the Team Manager

What is required?

• Working knowledge of relevant legislation including welfare benefits
• Experience of working with people with physical, learning or sensory disabilities and their carers
• Experience within a similar role
