Responsibilities:
...- 1 x Care Manager (Social Worker/Occupational Therapist) required to work within busy Locality Team (Urban) to
- provide cover and to offer operational resilience and support LSI process with local care provider and completion of
- related assessments and provision of supports and equipment etc.
- (OT/SW) candidates will be considered - although service has a preference for OT
- local hybrid working arrangements currently in place ; workers must also be available to undertake essential visits
- within domestic addresses/ individual properties as required.
