Role Overview:

Based in the Transfer of care hub at the Royal Oldham Hospital as part of an Integrated Discharge team, you will provide a personal, first point of contact for the Integrated Discharge team, for all customers, both general public and professionals requiring advice and guidance regarding discharge from hospital and local bed based services, safeguarding concerns or information and advice on statutory and voluntary agencies and other local activities.

Key Responsibilities:

  • To provide an information, advice and enquiry service to professionals from all services vulnerable adults, older people and their families and carers, across the Borough about the discharge process.
  • To support hospital discharges by restarting packages of care
  • To ensure that GDPR is adhered to when processing data and maintain records in accordance with policy and procedure including uploading discharge referral requests and liaising with professional on patient status.
  • To contribute towards the collation of data for customer satisfaction and other surveys and audits

Key Requirements:

To succeed in this role, you will need as a minimum:

  • Excellent communication and interpersonal skills are required to effectively advise and support professionals and members of the public with confidence and are able to challenge where appropriate based on evidence.
  • Be IT competent as all work in computer based.
  • Excellent organisational skills will be needed to agree, develop and manage the provision of appropriate business intelligence; ensuring managers have access to appropriate and timely information about their service and Oldham.
