About the Service
Fallings Heath provides excellent person-centred support for four young adults with learning disabilities and complex needs in a quiet, residential street.
In your new role as a Care Service Coordinator, you will:
- Manage the team to ensure that Care Services are organised and delivered safely and effectively
- Effectively communicate with Care Workers, including regular team meetings and supervision.
- Ensure effective operation of absence management process is followed.
- Ensure quality through appraisal, supervision, audit, spot checks, observations, customer feedback.
- Participate in the Out of Hours Service on a rota basis.
- Undertake MAR chart and Logbook Audits and action any discrepancies and learning outcomes with colleagues / teams.
- Ensure implementation of new care packages or changes to care, including need and risk assessment, care planning, completion of all support plan documentation.
- With support from the People team, investigate incidents, in line with policies and procedures, including missed calls, accidents and incidents, complaints or safeguarding, medication errors.
- Accept all referrals where safe to do so, and record all refused referrals, ensuring accurate, timely, recording of referrals, including increases, decrease and ending of care.
- Develop and maintain effective relationships with statutory commissioning teams.
As a Care Service Coordinator, you will ideally have:
- Excellent knowledge and understanding of CQC compliance/legal requirements
- A sound understanding and experience of the planning and management of homecare.
- Level 4 Qualified, or working towards
- Working knowledge of IT systems, including Microsoft Office, and able to learn care planning system
- Passionate about providing good, safe, effective care.
- Strong organisation and planning skills, and ability to work well and accurately under pressure
- Proven record of performing to targets while maintaining quality in homecare
- Good understanding of how to use systems and processes to support service delivery
