This post is to join a team of existing Case Managers and Assistant Case Managers at CA Case Management Ltd, part of the HCML Group. CACM is an independent Case Management company specialising in the provision of high quality Case Management services to clients with brain and spinal cord injury and long term neurological disabilities. As a Case Management Assistant you will manage a small caseload under clinical supervision and support with the management and development of Support Workers, patient communication and rehabilitation reporting.
This role would suit people who have worked as a disability analyst, support worker, carer, rehabilitation assistant or similar.
Please note this role is ideally being offered on a part time basis of 24 hours per week and the salary is 24,375 full time equivalent.
About the Role
The Case Management Assistant is a varied role whose responsibilities will include:
- Manage a case load of clients under the supervision of a case manager
- Meet with clients and their families on a regular basis.
- Work in accordance with relevant legislation, health care best practices and internal policy and procedures.
- Work with clients and support teams to create care and risk management plans.
- Work with support teams to ensure best practice and understanding of and adherence to care plans and risk assessments at all times.
- Undertake support worker supervision , reporting back to the case manager.

Essential Skills
- Minimum 5 years working experience in a disability related or rehabilitation service
- Minimum of 3 years experience working as team leader / senior support worker
- Experience in working with adults and children with neurological conditions, traumatic brain injury / Cerebral Palsy and other complex conditions.
- Knowledge of CQC registration requirements for health and social care providers.
- Knowledge/ skills in care and support planning and implementation
- An understanding of rehabilitation and goal planning
- Knowledge / skills in risk assessment and management plans.
- Diploma/ NVQ Level III or other Health and Social Care Qualification (or willing to obtain this qualification)
- Full UK driving license with access to own vehicle and be confident to drive across East Anglia and into London
