Job description
Provide personalised one-to-one and group-based support to children and young people referred for support. Develop trusting relationships, giving people time and focus on what matters to them. Provide non-judgemental support and work from a strength-based approach focusing on a persons assets. Take a holistic approach, based on the persons priorities and the wider determinants of health that impact on their health and wellbeing, such as family debt, poor housing, school issues, loneliness, and caring responsibilities.
For young people referred to secondary care for neuro diverse issues, work closely with the young persons family to support them on their journey to a diagnoses. Liaise with relevant school support staff to help create the right environment for the young person in which to learn. Contribute to the PCN Enhanced Access provision holding evening clinic / supporting group clinic every second week.
