Job Title: Clinical Governance Practice Lead Team: Clinical & Quality Base Location: Flexible working across Local Care Direct sites (primarily Leeds) Line Manager: Director of Clinical Governance & Quality Hours of Work: 22.5 hours per week Staff Group: Clinical DBS Check Level Required: Enhanced with Child & Adult Workforce & Barring Vaccination Requirement Statement: Covid19 double vaccination is recommended. This job has been rated as Medium Risk for requiring Hepatitis B immunisation, vaccination is recommended, therefore Post Holders will be offered a Hep B immunisation. Flu jab recommended & will be offered. Job Purpose: To work with the Director of Clinical Governance & Quality (Direct of CG&Q) and Clinical Governance & Quality Lead (CG&Q Lead) to lead the development & implementation of a system of Clinical Governance as part of an integrated organisational governance framework. Supporting the team with delivery of the patient safety incident response framework & promoting an open learning culture across all services provided by Local Care Direct (LCD) will be a key area of work. Specific responsibility for leading the improvement of clinical performance and practice identified through quality monitoring systems. KEY PRINCIPLES Deputise for the Director of CG&Q in their absence. Deputise for CG&Q Lead in their absence. Responsible for Clinical Governance procedures (significant event reporting, complaints management & incident reporting) ensuring these meet NHS and contractual requirements. And are efficient in capturing, recording and securing information & effective in identifying learning outcomes, action planning and identifying changes to services & service delivery. Developing & implementing effective methodologies & tools for auditing of clinical practice in line with best practice & contractual requirements. Responsible for promoting team working across LCD & ensuring teams adopt a multi-disciplinary team approach to complaint & incident resolution & work effectively to identify & minimise risk. As a designated Caldicott Lead, ensure these principles are promoted, understood & adhered to and that the Caldicott Guardian is fully supported. Provide Clinical Governance oversight in the development of care pathways & service development. Work with LCD stakeholders & business partners to ensure integrated Clinical Governance across patient care pathways. Work with other members of the clinical team to provide effective clinical supervision & support programmes. SPECIFIC RESPONSIBILITIES Clinical Governance, Risk Management & Effectiveness To be a Clinical Lead for management & resolution of complaints & incidents. Provide recommendations to the Director of CG&Q for improvement actions arising from reviews of complaints / incidents. Responsible for ensuring investigation techniques are timely, rigorous, taking an approach using Systems Engineering Initiative for Patient Safety (SEIPS), that is learning-centred. Develop response & resolution arrangements for complaints / incidents that have not caused patient harm. Responsible for ensuring appropriate consent mechanisms are in place before sharing any information regarding complaints / incidences. Work with the CG&Q Lead to provide the Director of CG&Q with a report relating to number of, and status of complaints and incidences & clinical risks for LCD Board & commissioners. Provide the Director of CG&Q with detailed reports & action plans for any new complaints / incidences received. Provide details to the GMC, NMC, HCPC, & attend Fitness to Practice Meetings. Arrange / attend local resolution meetings as & when required. Liaise with agencies & stakeholders where necessary & in accordance with GDPA & LCD Policies & Standards. Ensure that NHS & local complaints procedures are regularly reviewed. Adopt an integrated approach to Clinical Governance management across care pathways, working collaboratively with other providers as appropriate. Ensure that care pathways, policies & processes reflect best practice. Contribute to the development of care pathways & service development (inc tenders for new business). Support HR with clinical investigations. Clinical Supervision, Audit & Professional Development Develop & implement clinical practice audits in line with professional best practice & contractual requirements. Analyse audit data to identify the educational & training needs of clinicians. Assist the Director of CG&Q to provide audit reports to stakeholders as required. Identify / develop learning opportunities / tools for clinicians to support their continuing professional development. Work with Education Faculty colleagues to support training programmes. Provide support for LCD staff / clinicians and patients / patients families when required, including outside recognised social hours, due to the nature of organisational operating hours. Process Management Responsible for providing Clinical staff with feedback from Quality monitoring processes. Provide feedback to the Director of CG&Q on poor performing clinical staff (including both self employed & agency staff) about whom we have concerns. Develop processes to respond to actions identified from Quality monitoring processes. Develop processes to respond to actions identified from Clinical audit & develop support plans for clinicians. Clinical Caldicott Lead The role of the Clinical Caldicott Lead is to safeguard & govern uses made of patient information within the organisation as well as data flows to other organisations. The Clinical Caldicott Lead is a member of the Organisations Information Governance Group & will need to work closely with the other members of the group. The Clinical Caldicott Lead is supported by the Caldicott Guardian. Patient and Public Involvement Implement & monitor processes which listen to user opinion & allows the organisation to learn from their experience. Arrange / attend local resolution meetings as & when required. Openness Promote a blame free culture to ensure that mistakes are reported / discussed & lessons learnt. ADDITIONAL RESPONSIBILITIES Team Set clear objectives for Direct Reports which support the delivery of plans & monitor these within the agreed performance management framework, taking appropriate & immediate action where work performance falls below the defined standard for the role. Carry out PDRs for all direct reports. Promote effective team working across clinical & non-clinical staff. Promote team development.Engagement & Relationships Engage with services who seek to support a reduction in health inequalities. Act as an ambassador for the organisation & build effective, mutually beneficial relationships stakeholders. Develop strong clinical networks & relationships at local, regional & national level. To communicate effectively on clinical issues across the organisation & provide written & verbal reports as required to a variety of internal & external audiences. To represent LCD at formal meetings required by its contracts. Additional The post holder will be required to promote & adhere to all Company policies & as a clinician & manager will have specific responsibilities relating to: Standing Financial Instructions Equality and Diversity Health and Safety COMMUNICATIONS & RELATIONSHIPS To ensure agreed lines of communication and reporting are implemented within the team to support all employees understanding LCD business plans, objectives, priorities and changes. To promote effective cross-functional working & that the operational team supports the achievement of objectives, standards & protocols of other LCD areas. To attend meetings representing LCD & engaging in discussions appropriate to the role & level of the post. To provide regular reports on the performance & development of the service, as required. PATIENT CARE To ensure that the quality of patient care is firmly at the centre of the organisations objectives and vision. To ensure the management & monitoring of the highest standards or safety & quality in the delivery of healthcare & related services, and manage risk within LCD. FINANCIAL RESOURCES To ensure financial policies & procedures are maintained & the budget is adhered to and that best value for money is achieved. To provide business cases as required. CONFIDENTIALITY To operate & ensure the team operates within information governance protocols. Ensure business confidentiality is maintained at all times. TRAINING To complete the defined company induction programme & core training for this role & mandatory refresher training thereafter & ensure that accurate records of training are maintained. To ensure staff receive an annual Performance Development Review incorporating the development of a development plan. To seek feedback on their leadership. INFECTION PREVENTION & CONTROL Healthcare workers have a duty of care to patients and are expected to comply fully with the companies Infection Prevention & Control (IP&C) policies and attend/complete IP&C training, for personal and patient safety and for prevention of healthcare-associated infection. SAFEGUARDING Individuals have a duty of care to children & vulnerable adults that they come into contact with whilst at work to safeguard & promote their welfare and to work to prevent, recognise, respond, appropriately refer & record potential neglect or abuse, adhering to the companies policies & procedures. MANAGING SELF Ensure that relevant professional practice is evidence based & current. Ensure that continual professional development is maintained & facilitates the ability to provide care to a high standard. Ensure that mandatory training is up to date & participate in training as appropriate. Effectively utilise time management, organisational, co-ordination & if required - management skills. Identify own professional development needs through Personal Development Reviews.
