Job description

We’re looking for an experienced Community Nurse passionate about End of Life care.  You will join our Community Nursing Service in a Clinical Lead | End of Life Lead Role.   You will support the BaNES locality to help us continue to change things for the better. The community nursing team in BaNEs is a 24hr nursing service.

Main ResponsibilitiesWithin this role, you will:  


Support community colleagues in and out of hours to develop clinical skills and knowledge by working in practice with colleagues who require support in developing/enhancing End of Life skills 
Work with Clinical Leads and Training Learning and Enterprise to develop a clear training/skills pathway for End of Life training for newly qualified nurses, HCA’s/Assistant Practitioners/Nurse Associates, Registered Nurses and therapist.
Provide a quality learning environment for all members of staff and students maximising their opportunities for professional development and career advancement.
Provide information, advice and guidance about education and learning opportunities to any staff member and direct them to other team members where necessary.
Take responsibility for ensuring personal development in order to support the development of evidence based and best clinical practice.
Promote and actively participate in Clinical Supervision and safeguarding supervision as appropriate
contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
implement, monitor and maintain high standards of care delivery at all times.

deliver our high service standards, sharing best practice, and actively improving the way we work. 

The Ideal CandidateYou will be a NMC/1st level registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. You will have a passion and enthusiasm for driving and developing gold standard End of Life care. It is expected that you should have strong communication and organisation skills and a flexible approach which will ensure you deliver positive experiences to patients at every stage of their healthcare journey.  You will have a full UK driving license with access to a car for work purposes.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
