Clinical Pharmacist

Trident Healthcare Solutions are a recruitment partner of Frimley Health NHS Trust which incorporates Wexham Park Hospital, Frimley Park Hospital, Farnham Hospital and Fleet Community Hospital.

We are seeking Band 5,6 and 7 Clinical Pharmacists to work in a locum capacity within the trust.

Trident Healthcare Solutions have Pharmacist positions based at both Frimley Park and Wexham Park Hospital.

you must hold a valid GPHC

Responsibilities:
- Review and interpret medication orders and prescriptions to ensure accuracy and appropriateness
- Collaborate with healthcare professionals to develop and implement patient care plans
- Provide drug information and education to patients, caregivers, and healthcare staff
- Monitor patients' medication therapy for effectiveness, safety, and adherence to treatment guidelines
- Conduct medication reconciliation to identify and resolve any discrepancies or potential drug interactions
- Participate in clinical rounds and provide recommendations for optimizing medication therapy
- Document interventions, assessments, and recommendations in the patient's medical record

Experience:
- Minimum of 2 years of experience as a Clinical Pharmacist in a hospital or clinical setting
- Strong knowledge of anatomy, physiology, and pharmacology
- Proficiency in patient care practices and medication administration techniques
- Experience working in an ICU or critical care setting is highly desired (however not essential)

Skills:
- Excellent communication skills to effectively interact with patients, healthcare professionals, and other stakeholders
- Strong attention to detail and ability to analyze complex medical information
- Ability to work independently as well as collaboratively in a multidisciplinary team
- Proficient in using pharmacy software systems for medication order entry and documentation

Job Type: Full-time

Salary: £26.00-£35.00 per hour

Expected hours: 22.5 – 37.5 per week

Schedule:

• 8 hour shift
• Monday to Friday
