Job description

We are looking for somebody with a friendly demeanour, and excellent interpersonal skills which enable you to work well with other healthcare professionals.
If you are a confident individual with the ability to use your own initiative and problem solve, you could be a great asset to our Pharmacy team.
To work within the locality pharmacy team and provide an effective and integrated pharmacy Medicines Optimisation service to inpatients in community hospitals and patients in the community where required.
To support and work closely with the clinical pharmacists working in the localities across South Devon and in the Pharmacy Department of Torbay to ensure safe and seamless patient care between healthcare settings.
As part of the Medicines Optimisation team; provide an effective, integrated ward-based pharmacy service.
Undertake medicines reconciliation reviews on all newly admitted inpatients, liaise with ward teams to ensure patient safety, manage an effective system to ensure efficient and economic provision of stock, support the dispensary and other areas of pharmacy to ensure timely provision of medicines for patients, undertake mandatory audits and data collection, assist in the training and development of pre-registration technicians and other pharmacy staff.

Brixham Hospital is a 20 bedded Inpatient unit for people from 18 years of age. Our main services user group is over 50 years for rehabilitation for a variety of conditions. We also provide support for End of Life care and complex discharges. The unit has an active Shared Governance group and have most recently achieved the Gold award for Pathway to excellence.
The unit is also the first full Health and Well Being Hub within the Trust and prides itself on having Primary and Secondary care on site as well as Voluntary services and an out patient department.
This is a new post for Brixham Hospital and we are very excited about the benefits it will bring for our services user as well as complementing our Multi disciplinary team. The post holder will also be part of our local governance as well as wider governance with the acute hospital. Supervision will be from the Medicines Optimising team as we recognise the need for close links and networking with the acute and other community sites.

  • As a qualified Medicines Optimisation Technician (formally Medicines
Management), ensure the provision of an effective, integrated ward-based pharmacy service to the community hospitals. In conjunction with the designated clinical pharmacist, and working closely with ward staff, develop a ‘team-based’ approach to ensure involvement throughout the patient journey, at key phases of the stay: admission; inpatient monitoring; and counselling and discharge. Demonstrate commitment to the integration of services for the benefit of patients
  • At ward level, undertake medicines reconciliation for new patients on
admission, referring discrepancies to the pharmacist or ward staff for
resolution; where appropriate; enter details of all medicines prescribed into the electronic care planning summary where available
  • Assess patients’ own medicines against agreed criteria to determine
suitability for continued use on the ward; where appropriate, approve for re-use by nursing staff by endorsing the prescription chart. Ensure the safe movement of patients’ medicines on transfer to other wards or hospitals
  • Issue dispensed prescriptions to patients or to their representatives, and ensure the provision of an appropriate level of information and advice to ensure they are able to use their medicines safely and effectively, referring to a pharmacist where appropriate
