Job Purpose To provide an advanced level of clinical expertise in the assessment, diagnosis and management of highly complex patients within primary care and the community interface service, providing an expert resource within primary and community care and implementing evidence-based practice. To work independently and autonomously managing a varied and complex caseload requiring expert clinical reasoning skills. Working across CityCare as a lead clinician within MOSAICs pain specialty and providing support to MOSAICs orthopaedic and MSK pathways when needed. To work in conjunction with key partners in planning, development and delivery of specialist services, crossing professional and organisational boundaries. To initiate, facilitate and participate in research and audit within the pain specialism. To provide clinical education, training, supervision and support to other staff and students within specialty area. Where required, to provide clinical education/teaching within local higher education institutes. To support the Head of Service and Team Manager in planning and service development. To work collaboratively across organisation boundaries in the development of evidence-based speciality services. Dimensions To provide a minimum of 50% of working time in the delivery of expert clinical care of complex cases and mentoring staff within (but not exclusive) to pain speciality. To provide training, clinical supervision, mentorship and leadership to clinical staff working within specialty, but also the wider organisation and integrated care system To work collaboratively and across organisation boundaries in the development of evidence-based speciality services. To be responsible for the delivery of the MOSAIC service for clients referred by GPs, other health professionals, social services and self-referral. To supervise, educate and assess the performance of undergraduate and post graduate physiotherapy students as required, ensuring to the standard of practice and teaching meets the standards set by the degree level qualification. To work in an extended scope including but not exclusive to: Orthopaedic triage of secondary care referrals Low Back Pain triage of secondary care referrals Clinical assessment and treatment planning in Integrated Clinical Assessment and Treatment (ICATS) clinics Referral for diagnostic examinations (MRI scan, x-ray) Administering and evaluating outcome of injection therapy To monitor and maintain performance of clinical sub-team of physiotherapists, non-medical prescribers, pharmacists, Pain Consultants and other sub-contracted clinicians. To lead and manage research projects and project teams and act as a resource for research and innovation in AHP and patient related issues, with particular focus on (but not exclusive to) MOSAICs pain speciality. Key Responsibilities Clinical To be professionally and legally responsible for all aspects of own work, ensuring a high standard of clinical care for the patients/clients under your management. To be a lead clinician within (but not exclusive) to pain speciality. To undertake the comprehensive, advanced assessment of clients, including those with complex and/or chronic presentation using investigative and analytical skills and to formulate individualised management and treatment plans, using expert clinical reasoning and using a wide range of treatment skills and options to devise a specialised programme of care. To demonstrate highly developed physical skills, with high levels of hand and eye co-ordination. To record clinical findings on the appropriate paper and computer records. To accept clinical responsibility for a designated caseload of clients and to organise this effectively with regard to clinical priorities and use of time. To provide highly specialised advice, teaching and training to other members of CityCare and wider local healthcare system regarding the management of clients with problems related to chronic pain conditions. To train and supervise more junior staff and post-graduate students. This will include the use of formal appraisal documentation and accreditation as a Clinical Educator. To supervise, educate and assess the clinical performance of post-graduate students as required ensuring the standard of practice and teaching meets the standards set by the masters level qualification. To communicate effectively with clients/carers to agree treatment goals and maximise rehabilitation potential and to ensure understanding of the condition. To promote collaborative relationships with patients and carers taking into account their needs and wishes, and respecting their diversity, to improve patient outcomes. To informally assess capacity, gain valid informed consent and work within a legal framework with clients who lack the capacity to consent to treatment. To provide spontaneous and planned evidence-based advice, teaching and instruction to clients, carers and other professionals to promote understanding of the aims of the service and to ensure a consistent approach to client care. To demonstrate the ability to receive complex information and demonstrate critical thinking, reflection and analysis to inform practice. To take a lead role on clinical practice issues, raising the profile of the service and offering specialist advice to assist in the implementation of service improvements. To implement and evaluate the use of outcome measures as endorsed by clinical and professional guidelines and standards and clinical indicators in order to monitor clinical effectiveness, in liaison with other healthcare staff. Professional To work as an autonomous professional. To act as a role model to provide advice/support to all members of the service and wider Nottingham CityCare organisation on clinical practice issues within the clinical area. To ensure own continuing professional development and contribute to Clinical Governance issues within the services and participate in CityCares appraisal process. To provide evidence of ongoing and appropriate CPD, as well as participation in the appraisal process. To continually assess and monitor risk to self and others practice and adapt process and practice as and when indicated in a responsible manner. To provide timely accurate data as required by the service and CityCare. To maintain patient records in line with CityCare policies. To ensure that work is carried out within Health & Safety guidelines and policies and have due regard for the safety of others in the work environment. To report all incidents/near misses appropriately. To represent CityCare externally and participate in the services marketing strategy when required, by developing local and regional networks and conducting presentations to a wider audience e.g. public sector and voluntary agencies. Organisational To support the Locality Lead, working with Team Manager, to implement targets and objectives to assist with the ongoing development of both the Nottingham CityCare and MOSAIC service. To take a leading role in service evaluation. To deputise in the absence of the Clinical Services Manager. Responsible for day-to-day supervision of a group of senior clinicians within the team. To attend team meetings and meetings with other agencies as required. To ensure the health & safety of staff and clients at all times in accordance with Health & Safety legislation and Trust policy and procedures. To adhere to all other CityCare Policies and Procedures. To manage the budget of allocated projects (where the Clinical Specialist is the Project Lead) with responsibility for signing timesheets, equipment orders etc. To undertake any other duties where required Leadership To collate data regarding own clinical intervention and that of other clinicians, and use data made available through NHS systems to inform service development. To act as a resource for clinical staff across the organisation and partner organisations, via mentoring, clinical supervision and individual case critical incident analysis, to enable those staff to effectively manage patients within specialty. To lead and participate in the development of protocols and care pathways to facilitate best practice within MOSAICs pathways, in line with local and national strategies. To ensure consistently high standards of interventions within the pain specialty across all areas of CityCare and the wider Integrated Care System by promoting and implementing evidence based practice. To demonstrate leadership skills that can influence service improvements within the specialist field. To be an acknowledged specialist who develops and shares innovative practice and service delivery models.
