A leading Berkshire charity is now looking for a Registered Nurse (RN Adult) or Paramedic experienced in Palliative Care to join the team as a Clinical Specialist, leading the team in delivering comprehensive community response / Virtual Ward services to local people with life-limiting illnesses such as late-stage cancers, organ failure, and degenerative conditions.
The charity provides a full range of adult palliative and end-of-life services both at their state-of-the-art, "Outstanding"-rated Maidenhead site and within patients' own homes. With their specialist inpatient, day therapy, bereavement support and new Virtual Ward services, the charity is able to provide safe, agile, and fully holistic care wherever it is needed.
The Virtual Ward service was set up in late 2022 as a means of enabling patients with complex symptom and psychosocial needs to continue receiving specialist care in their preferred place, allowing them to remain in familiar surroundings for an improved quality of life.
As a Clinical Specialist Practitioner, you will provide the day-to-day leadership of this service, managing a highly-skilled MDT with consultant-led oversight. You will also conduct the assessment and triage of new referrals, plan advanced interventions, and act as an expert source of clinical knowledge and advice for patients, their loved ones and fellow healthcare professionals, all so that the service can remain as responsive and person-centred as possible.
An excellent learning & development team will support you to further develop your skills within this specialism, and you will be able to participate in internal and external educational activities for junior team members, patients/carers, and other groups as well.
This is a permanent, full-time (37.5h) position for a Clinical Specialist Practitioner (Nurse or Paramedic). You will need access to personal transport in this role due to travel requirements.
Person specification :
* ( Essential ) NMC registration as a Registered Nurse (RN Adult) / HCPC registration as a Paramedic
* ( Essential ) Clean UK driving licence and access to personal vehicle
* ( Essential ) Evidence of broad clinical experience to include palliative care
* ( Essential ) Experience as a Clinical Nurse Specialist / Specialist Paramedic Practitioner / equivalent
* ( Desirable ) End-of-Life Care qualification
* ( Desirable ) Non-medical prescribing qualification
