Job description
The post holder will work within the Community Care team (previously Social Care and Children’s Directorate team). They will be responsible for contributing to and supporting the development of services in a range of areas across all adult Programmes of Care through the work of the SPPG and PHA including leading on specific regional and local projects. This will include detailed analysis of issues and challenges to service user pathways, including in-patient treatment, residential and nursing home care, the prevention of harm, and the development of community-based services to support service user pathways/Networks.The post holder will provide advice and support to Regional and local Networks, Local Commissioning Groups, and other regional groups as required. The post holder will participate in the out of hours on call rota as required. In all duties the post holder will be expected to act with integrity, consistency and purpose in, enhancing individual and community health and social well-being and reducing inequalities. .

Have at least three years post qualifying experience working at a senior management level in health and social care services in the statutory/voluntary or independent sectorA professional qualification in a Health or Social Care profession
