An exciting opportunity has arisen in the Older Adult CMHT in Bolton for a Community Based Discharge Care Coordinator.
The role will require the successful candidate to facilitate discharges for a caseload of inpatients based in Woodlands Hospital under all relevant legislative and care frameworks: CPA, MCA (2005), MHA (1983), and The Care Act (2014).
This role is open to Registered Social Work colleagues, Registered Mental Health Nurses and Registered Occupational Therapists with relevant knowledge and experience for the role.
Under the line management and clinical supervision of The Older Adults CMHT, the successful candidate will coordinate timely and effective, initial CPA meetings upon admission to hospital and discharge CPA meetings at appropriate times during a person’s hospital stay. Commencing discharge planning at the point of admission in collaboration with families/carers and
social care where appropriate. Undertaking/ overseeing appropriate assessments and liaising with social care staff, funding panels and future care placements.
You will be expected to complete assessments under the Care Act (2014) and subsequent support plans, alongside Trust Approved risk assessments (STARV2) and CPA care plans; and handover to your colleagues in GMMH or The Local Authority for seamless transition from inpatient to community care.
Alongside this, there is also an expectation that the successful candidate will support the OACMHT Duty Rota as required.
This is an exciting opportunity to develop and embed the discharge pathway in the OACMHT and improve service user experience.
