Job description


The Integrated Front Door Team is a well-established team of community nurses based at the Front Door of the hospital working in collaboration with Health, Social Care, Mental Health, GP, Housing and Volunteer services.
Opportunities have arisen to develop a community nurse within this unplanned service under the umbrella of Walsall’s innovating Walsall Together.
You must hold excellent community pathways and service knowledge and a variety of clinical community experience to be considered for this opportunity within the team. You will have responsibility for a varied workload of rapid interventions and you will receive referrals directly from ED, AMU and other Front Door Acute services, working initially under the direction of the Clinical Team Leader.
You will be working with a wide variety of Adult Services to develop and support this client group to bring care closer to home and to remain in their homes with enhanced services or access to Clinical Community services to avoid hospital admission or to expediate hospital discharge.
You will need to have at least two years’ experience of working within the Community services at Band 5 level, work flexibly to meet the demands of the service and be able to work effectively with partnership agencies at Social Services, Acute Trust and third party agencies.
The service provision is delivered between the core hours of 08:00 and 20:00 across 7 days. Applicants must be willing to participate in a rotational shift cover.
You will need to be able to work in an autonomous way and facilitate effective communication between ward staff, the integrated discharge team and social care team, to promote a culture where transfers of care between primary, community and secondary services are safe, timely and well co-ordinated for patients who no longer require acute hospital care.
Part of the role will involve working within the Community nursing teams proactively supporting and case managing a caseload of complex patients with a range of long term conditions, who are at high risk of unplanned admission to hospital or prone to unnecessarily long lengths of stay.
• You must be able to travel throughout the working day across the borough.

Construction of the new integrated critical care unit was completed in2018,extension of a Neonatal Unit and brand new, state of the art Emergency Department is in the process of being completed.

• Use skills and knowledge to assess the physical and psycho – social needs of the patients in their own homes within 24 hours of hospital discharge Including observations, medication checks, and skin checks.
• To work with the hospital Discharge Team to facilitate safe, timely and appropriate transfers from wards for patients who are known to place based teams
• Anticipate short and long term care needs and develop appropriate care plans for patients in order to prevent unnecessary readmissions to hospital including emergency passports.
