Job description

An exiting opportunity has arisen to work as a Community Health Care Assistant in the Frailty Virtual Ward. The Frailty Virtual Ward provides assessment and treatment of people in their own home who become suddenly unwell and would normally be admitted to hospital. The virtual ward provides an alternative to hospital admission as well as supporting discharge, improving outcomes and reducing the risk of hospital acquired adverse events including deconditioning. The virtual ward will work alongside and communicate with existing providers to ensure the patient is safely cared for within their own home.
The post holder will be responsible for directly supporting The Community Business Unit as part of, but not limited to, the frailty virtual ward team. The post holder will need to carry out a range of delegated duties including hands on delivery of nursing procedures, patient care as well as carrying out some administrative duties. The role may also require reablement and rehabilitative tasks under direction of a qualified physiotherapist or occupatinal therapist.
The duties will be carried out in the community setting under the supervision and guidance of the frailty virtual ward MDT; however the post holder will be expected to work alone with access to professional advice and guidance when required.
The post holder will provide vital support to the Community Business Unit with integrated seamless services within community settings working in partnership with local authorities, other Health Care providers, GP practices and the acute Trust.
The post holder will play a key role in supporting the community business unit with a range of interventions and nursing tasks which enable the patient to receive the optimum care health outcomes. There will be a need to understand and undertake administration and co-ordination tasks within the team as and when required. The post holder will work with minimal supervision, demonstrating the ability to use initiative and utilise IT systems.

Planning: Assist the planning of team workload and diary and schedule management with daily use of Emis Community IT system. Planning own workload with the team ensuring timely patient intervention.
Coordinating: Developing and maintaining effective systems and processes in line with the business unit and teams needs. Ensure accurate and effective records are kept. Assist in maintaining any schedules, appointments and visiting schedules as and when required.
Supporting: Support the team and service leads with the day to day delivery of a range of clinical interventions to patients within the community setting. Support the MDT in the development of any new ways of working that will ultimately improve patient care as and when required. Support the development and delivery of the community teams including the implementation of robust working practices processes, audit and lessons learnt.
This post will be based within a location that is part of Gateshead Community Services.
This post is to cover Monday to Friday 9am to 5pm. This post may include bank holiday working and may expand to cover out of hours provisions in the future.
