CALLING ALL PRE-REGISTRATION & NEWLY QUALIFIED MENTAL HEALTH NURSES INTERESTED IN COMMUNITY MENTAL HEALTH NURSING.
The Luton Community Mental health Hub have created Band 5Mental Health positions to support within the triage Multi-disciplinary Team
You will work in multi-disciplinary recovery based Mental Health services and will be a core member of the triage multi-disciplinary team, participating in the provision of high quality Mental Health meaningful interventions. The role includes working closely with our community partners, including GP, staff and voluntary sector. The team’s vision is to move work from request of support to making contact with service users in a timely manner.
The team is made up of a rich resource of multidisciplinary professionals who support psychological ways of working. The team includes Registered Nurses, Social Workers. Doctors, Psychologists, Assistant Psychologist, Occupational Therapists, Support Recovery Workers, Psychotherapists, Administrators and an Employment specialist as well as working closely within a designated locality with Primary care Mental Health link workers, the Local Authorities and voluntary services.
The post holder will be responsible for:
Attending MDT triage
Making Contact with the service user in a timely Manner following from Triage to screen for risk effectively and to complete initial assessment.
To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities.
Ensuring that the values outlined in the NHS Constitution are adhered to daily and any matters of concern are raised with the relevant Line Manager or through the necessary processes within the Trust. Responsible for delivering a compassionate, dignified and respectful service to
patients at all times.
Continually delivering and maintaining high standards for a defined caseload as agreed with Line Manager.
Ensuring national targets and standards are achieved. Building trust and developing relationships to ensure service user involvement.
Demonstrating a professional manner in all work carried out.
Maintaining accurate and timely records.
Ensuring equality and diversity values underpin the day to day working of the practice area.
Continuous professional development.
