Hays Healthcare is recruiting RMN's and Mental Health Practitioners (Band 5) as opportunities have arisen for Community Mental Health Nurses for our clients in York.

Your new role
The CMHT work with people who have severe and enduring Mental Health and social care needs. The CMHTs are integrated with the local authority, providing a multidisciplinary approach to the care provided. You will be responsible for providing holistic assessment, interventions, risk assessments and care plans for their caseload.
Main duties
- Assessment of health and social care needs
- Producing, monitoring and evaluating collaborative care plans
- Risk assessment and management
- Providing evidence based therapeutic interventions.
- Providing education advice, emotional and psychological support
- To promote choice in service user involvement in care planning and risk management
What you'll need to succeed
- Valid NMC PIN (essential)
- Right to Work in the UK (essential)
- Full UK Driving Licence & own transport (essential)
- 6 months experience in the role within the UK (essential)
- Proof of Immunisations to Measles, Varicella, Rubella, Tb Hep B (essential)
