Community Mental Health Team Social Worker

£35 - £38 per hour Umbrella company (dependent upon experience)
A Locum Social Worker is required to join one of RCTs' Community Mental Health team for an initial 3-6 month contract. This is predominantly a hybrid role whereby you can work between home and their on site offices. We are looking for someone who is able to commit to a full time contract of 37 hours per week. Office base for this role is Treorchy.
If you are a creative, enthusiastic Social Worker with a sound value base, who has experience of working with people who experience co-occurring mental ill health and substance misuse Hoop Social Work would love to hear from you.
In this role, you will need to be organised and able to prioritise competing job demands. You will also have a strong belief in empowering adults towards independence using the Recovery Model and be proactive in challenging discrimination.
You will work as part of a multi-disciplinary team and you will have good communication skills and the ability to form effective working relationships with a range of partners. You will be expected to be conversant with relevant legislation and concepts of best practice. Knowledge of the principals underpinning the Care and Treatment Planning approach is desirable. You should be able to demonstrate a comprehensive understanding of the issues facing people who experience mental ill health/ substance misuse and their carers.
Main duties and responsibilities in brief include:
- Holding a caseload
- Covering duty calls within office environment / support available if needed by duty worker while on duty
- Caseload, completing care and treatment plans / risk assessments and reviews
- Attending meetings online or in person occasionally.
- Attending appointments with drs/ sus
- Joint working with team members
- Monitoring visits
- Assessment clinic/ home assessments
There is regular professional supervision and you will also have the opportunity to become involved in peer supervision. An ongoing programme of training for all staff provides a diverse range of learning opportunities to meet the demands of the work and support individual professional development.
You must be suitably qualified (i.e. Diploma in Social Work or equivalent) and be registered as a Social Worker with the Care Council for Wales. An Approved Mental Health Professional qualification is desirable however not essential.
