Situated in Zone 5, Sutton is less than 30 minutes by train from central London, and south and south west London are easily accessible by public transport. There are 10 train stations which serve key neighbourhoods within the borough providing direct connections to London Victoria, London Bridge, London Blackfriars and St Pancras International. Sutton also has direct rail connections with Epsom, Dorking, Guildford and Horsham, as well as Wimbledon for an interchange with the London Underground and West Croydon for the London Overground. There’s also an extensive road network which provides close and easy access to the M25 and M23.

About the Role 1

Community Nurse - Adults with a Learning Disability

£39,264 - £47,040 (unconsolidated maximum £49,083) equivalent to NHS - Band 6/7

This is a really exciting opportunity to join our MDT of Nurses, Physiotherapists, Occupational Therapist, Speech and Language Therapist and Creative Therapists.

Reporting to the Team Manager for the Clinical Health Team, you will be required to deliver the best outcomes for the Health of Adults with a Learning Disability, working with Social Workers, Commissioners and other teams across the NHS. These roles will support our long term commitment to Sutton’s LD Strategy to help increase improve health, independence and social participation. You will contribute to NHS Continuing Health Care assessment and review. You will also work to maximise take up of healthcare services (including GP annual health checks) and reduce visits to accident and emergency departments and admission to hospital.

This is a really exciting opportunity to join a small, friendly, local team of varied Clinical Professionals working closely within our unique MDT of Learning Disability Nurses, Physiotherapists, Occupational Therapist, Speech and Language Therapist, Psychologists and Creative Therapists. A unique placement within the London Borough of Sutton, our team works productively with Social Workers and Commissioners across Health and Social Care with individuals, families, those in supported Living and residential placements. This specialist team assesses and/or treats individuals/groups who cannot access mainstream services despite reasonable adjustments. With good leadership we support our team with regular operational supervision and clinical supervision. We also provide training opportunities for further clinical professional development.

About You 1
  • Qualified Community Nurse with an in-depth knowledge and understanding of the health needs of adults with a learning disability.
  • The ability to show great compassion and empathy when caring for patients with specific and sensitive needs.
  • Creative in using different methods of communication to promote health and wellbeing.
  • Skilled and proficient at engaging with the people who have a range of communication needs.
  • Can deliver a variety of evidence based clinical interventions, using a range of evidence based resources and tools.

Given the demanding nature of the role and the spectrum of patients, we would expect you to possess excellent communication and listening skills, while demonstrating sensitivity towards the specifics of each unique situation. This role requires self-reliance and the ability to apply your knowledge and judgement as quite often you’ll be working alone and won’t have anyone immediately on-hand to assist you.
