The Bridge Project is looking to recruit a resourceful, well-organised, self-motivated, and pro-active individual to become our Community Health Partnership Coordinator on behalf of Affinity Care Community Partnership, otherwise known as ACCP.
The role will spend approximately 3 days per week working on a specific Community Centre Health Development Project funded by Core20+5 reducing healthcare inequalities monies. The remaining two days per week will be focussed on more general community partnership support, for example, administering small grants rounds, supporting project providers, managing the CP’s social media presence, and responding to enquiries.
The role involves & requires:
Experience of community and / or health development
The ability to engage, communicate and collaborate effectively with a wide range of stakeholders, including the public and a range of statutory and voluntary organisations operating within the locality.
Strong organisational skills to develop, manage and evaluate projects.
The ACCP area covers the communities served by the following GP practices:
Shipley Medical Practice (including Westcliffe Medical Centre)
Thornton & Denholme Medical Practice
The Willows Medical Centre
Haigh Hall Medical Centre
Sunnybank Medical Centre (including Cowgill Surgery)
It is essential that you have a driving licence with access to a vehicle, due to the geographical reach of the ACCP area. A willingness to undergo a standard DBS check is also a requirement of the post.
