Job summary
An exciting opportunity has risen for a Band 4 Care Coordinator to join our Community Therapy Service Brighton.
We would like to hear from you if you feel passionate about your work. The post holder should be a creative and clear thinker, with administrative experience and therapy knowledge. Have excellent skills with technology to help manage our large Community Therapy Team caseload and to monitor through put and the quality of the service we provide.
Our aim is to maximise opportunities for people in our communities to be as independent as possible in what matters to them, living full and active lives within their families, social networks, and workplaces.
Our teams work flexibly to provide a seven-day service, 365 days a year, between 8am and 6pm.
The post holder will provide prompt, efficient co-ordination and administration support to the service. This role can be offered flexibly to be based in any of our offices (Brighton General Hospital and Hove polyclinic).

Main duties of the job
The focus of this role is providing prompt and efficient co-ordination and administration support to a multi-disciplinary team (MDT).
You would be involved in:
  • Caseload and case management
  • Supported discharges from Acute and rehabilitation units
  • Community and UCR referrals for therapy input
  • Phone calls with patient, staff, acute hospital, UCR, care agencies etc.
  • Part of multidisciplinary team
  • Through close liaison with the Clinical Leads and duty coordinators , to manage the Team waiting lists, allocating cases, contacting clients, booking them into appointments
  • To ensure that an accurate and up-to-date patient record is maintained by updating System One as necessary.
  • To produce reports to team managers in conjunction with the service manager that enable monitoring of clinical work and strategic development.
  • Using a shared email inbox to improve communication.

You will be the point of contact for service users, their families, and carers. This will include enquiries about anticipated visits, providing updates, and listening to their concerns.
You will sign-post referrals to MDT members and other services as appropriate while monitoring, tracking, and reporting on progress within the patient pathway. This will involve escalating any issues or delays to the Operational Team Lead for appropriate action.

Qualifications
Essential
  • GCSE passes at Grade C or above or equivalent experience
  • NVQ level 3 or equivalent (health and social care or business administration) or be willing to undertake

Desirable
  • IT/Administration Qualification

Experience

  • Evidence of consistent pattern of learning from education, training and experience

Desirable
  • Experience of working in a health or social care setting
  • Previous experience in an administrative role
  • Experience of working within multi-disciplinary teams

  • Considerable experience working with the public

  • Able to maintain confidentiality and deal with sensitive information
