Adult Complex Care Nurse
Complex Care Nurse Main Duties and Responsibilities:
- To act as a service lead in all aspects of complex and personal care and support for our service users.
- To provide nursing care and support which meets TSF Healthcare Solutions standards, regulatory requirements, the Compassion in Practice 6C s and adheres to the NMC Code of Conduct and guidance within the NMC Scope of Professional Practice .
- To implement the organisations Safeguarding Policy, and ensure robust and effective Safeguarding Systems and Procedures are in place to ensure the safety of all our service users.
- To ensure that care and support is provided safely, efficiently and effectively, in a way that promotes choice and independence and respects the privacy, dignity and lifestyle of each service user.
- To assess complex and personal care needs for new and existing service users, and develop personalised care plans and adjust them as necessary.
- To review, monitor and record conditions, escalate cases and make referrals as needed.
- To maintain accurate and up to date records of all service users.
- To prepare reports for the Registered Manager and the Board of Directors to assist decision-making and to retain efficient and accurate records in accordance with organisational policy and legislative requirements.
- To work collaboratively with other healthcare providers such as General Practitioners, Social Workers, Occupational therapists, and Psychologists to ensure service users needs are met.
- To develop work schedules, assign tasks and arrange training for healthcare assistants as necessary.
- To work independently and as part of a team to keep service users safe and well in their own homes.
- To keep service users, their families and healthcare assistants fully involved and informed during the episode of care.
- To participate in health education and other health care initiatives within the organisation s service area.
- To provide Complex Care and support services (e.g. stoma care, catheter care, PEG feeding, wound management, nephrostomy care, tracheostomy care, complex medication administration, ventilation, end-of-life care) to service users in their own homes.
Complex Care Nurse Qualifications/ Experience:
- RGN Qualification is essential.
- Evidence of Current NMC Registration is required.
- A post registration qualification in a relevant area is desirable but not essential
- Full UK driving licence and access to a vehicle.
- Minimum of 1 years' experience in a Complex Care or similar environment.
- Clinical skills in Complex Care
- Complex case load management experience.
- Use and implementation of clinical guidelines, standards, protocols and care pathways.
- Infection control
