Job description

About the Department:
Critical Care Outreach Team (CCOT) sits within Lincoln County Hospitals’ Intensive Care Unit which is a modern, combined ICU and HDU. Since its inception in 2001, the Critical Care Outreach Team has evolved in its response to the needs of the deteriorating patient, providing a robust 24/7 service.
The Role:
In 2021, ULHT’s Critical Care Outreach Team (CCOT) obtained Advanced Clinical Practitioner status, a role that employs all elements of the HEE Multi-professional Framework and the four pillars of advanced practice. This trainee CCOT ACP post requires an enthusiastic, degree level registrant with recent extensive experience of managing the care of the deteriorating patient.
We are offering a 37.5 hour substantive contract for a trainee Critical Care Outreach ACP to work within this experienced team who are committed to delivering high quality patient care.
We seek an individual who has excelled within their area of practice and who has evidenced experience within an acute setting.
The successful applicant will hold a position at Lincoln County Hospital but there is an expectation to work cross site dependant on the needs of the overall service. This includes Pilgrim Hospital, Boston and Grantham and District Hospital.
Management of such patients is a challenging and demanding role. It is an expectation that the individual will work to an advanced level of clinical practice, exercising autonomy, complex decision making, undertaken with varying levels of risk, all of which hold professional accountability for decisions made.
The ideal candidate must have a passion for the deteriorating patient. With experience of multidisciplinary working and demonstrate excellent communication skills, they will be willing to develop their clinical and professional practice to meet the challenges of this exciting role.
We welcome applications from other Allied Health Professionals Ability to train as an independent prescriber is an essential attribute. Recent Critical Care experience / Critical Care qualification is desirable but not essential.
If employed as a trainee practitioner, there is an expectation that the ACP MSc is undertaken within a full time position to facilitate workplace learning.
If you already hold qualifications and experience as per the Band 8a Job Specification (attached), you will be considered to commence as a AfC Band 8a CCO-ACP subject to portfolio evidence and interview.
Our Trust is situated in the beautiful county of Lincolnshire and is one of the biggest acute hospital trusts in England, serving a population of more than 700,000 people. We provide acute and specialist services to people in Lincolnshire and neighbouring counties. We have an annual income of £643 million (20/21) and we employ nearly 8,500 people.
Our Board have recently agreed a new vision statement – “Outstanding Care Personally Delivered” – stating their ambition for our Trust to be among the best.
We also have a five year Integrated Improvement Plan setting out how we will achieve our strategic objectives, for patients, services, people and partners, the last recognising that our future success depends on our ability to ensure the Lincolnshire healthcare system is successful in achieving its ambition to help people live healthier lives and provide care closer to where people live.
Our latest CQC report increased our ratings for being effective and well-led from ‘Requires Improvement’ to ‘Good’. Our rating for caring remained as ‘Good’.
This is in recognition of the huge amount of work that our amazing staff have done to improve the quality of care for our patients in recent years.
We are committed to helping our staff balance their work and home lives and ensure they feel supported, valued and appreciated.
We would welcome and encourage informal visits.
