Location:                   This role is community based – working across the Doncaster area, in community venues, local service locations (locality based) and home working elements.

About the role
Alzheimer’s Society Doncaster have an exciting new partnership with Rotherham, Doncaster and South Humber NHS Foundation Trust which will be delivering memory and dementia support across the City of Doncaster.
We are looking for Memory/Dementia Advisors to work as part of a multi-disciplinary team which includes clinical professionals working together to improve outcomes for people experiencing memory issues and those diagnosed with dementia, their carers and supporters.
This role is community based and you will be attached to a locality team working in Doncaster. You will be community based, working out in the community, doing home visits, working at a locality base and at home. The service covers the hours of 8.30am -6.00pm and you will be required to take your turn covering the telephones with other members of the team.
The service will:
  • Triage referrals to the appropriate level of support providing a supported referral where appropriate
  • Work as part of a multi-disciplinary locality team providing information, advice and support for people with memory problems, people with dementia and any carers or supporters
  • Support people to maintain their independence, improving their sense of well-being enabling them feel in more control of their lives
  • Build relationships with a range of local contacts and network with health and social care professionals, which includes services provided by the new Keep in Mind partnership providing community activities and managed by Age UK Doncaster
  • Support other organisations to build their capacity to support people with dementia and carers by raising awareness through the dementia friends programme
  • Provide support face-to-face, on the telephone and by email
  • Deliver the Carers Support and Information Programme and the Live Well with Dementia Programme
  • Provide proactive contact to people to reduce potential for crisis

About you
You may already have been working in health and social care, want to change your career or be someone with an interest in this area of work who has just left further education. Alzheimer’s Society provide comprehensive training, with opportunities to shadow others in the role and work with our partners to build up your experience, our requirements are that you show you have an understanding of memory problems and dementia and the impact that has on people and their lives. 
Your ability to assess client needs along with the understanding of the need for client confidentially is essential to providing a valuable and worthwhile service. Due to the nature of the service, experience of safeguarding would be beneficial.
To be successful in this role you will;
  • Have excellent communication skills as well as strong organisational skills in order to manage your caseload of clients effectively within tight deadlines.
  • Be good at developing relationships and networking with professionals and organisations in Doncaster
  • Be empathetic and non-judgmental in your approach with a commitment to equal opportunity.
  • Be passionate about putting people with memory problems, people affected by dementia carers and supporters at the heart of everything you do, to make our service the best it can be
  • Have a sound knowledge of IT systems such as Microsoft Office  
  • Manage your time effectively connecting with your colleagues/team in locality bases, remotely, working out in the community and at home
  • Be able to travel independently across the City of Doncaster localities
